Well site was identified with well sign and E&S plan. Heavy rains causing visible E&S concerns at the time of inspection with sediment occurring on the road and flowing into streams. Requested Cambria DEP water quality personnel to perform an evaluation. Drilling activity was temporary halted until August 2, 2009. No IW discharge issues. Pit aligned with 20 mil plastic and had no freeboard issues. Filter fence located topographically down gradient of site pad.
1819594 Primary Facility 2009-08-06 Drilling/Alteration No Violations Noted Currently, well completed to target depth. Well site was identified with well sign and E&S plan. E&S concerns at the time of inspection with sediment occurring on the road. Site is being prepared for Fracturing of well. No IW discharge issues. Pit aligned with 20 mil plastic and had no freeboard issues. Filter fence located topographically down gradient of site pad had considerable amount of sediment in front of fence from previous rains. Well completed with 7¿ pipe reduced to 4¿ with shut off. Bull dozer and frac tank located on-site. Area behind tank was starting to show stress release breaks in the site pad. Labor on-site was going to move the tank in order to prevent site pad from collapsing.
1965270 Primary Facility 2011-04-13 Site Restoration No Violations Noted At the time of inspection, well drilled and in production. Well tag present. Brine tank appears stable. Site has been restored with at least 70% uniform vegetative cover. Site does not exhibit ES issues. Minor ES issues noted along shared access road due to construction of Krause 1 site (Spud called for 3/30/11), which are addressed accordingly in Krause 1 site inspection. At the time of this inspection no violations noted at this site.

No accelerated erosion or sedimentation issues observed in relation to well site. Acess road stable, ruts beginning to form. Department recommend Operator maintain access road to prevent a potential pollution even from occurring.
