1848343 Primary Facility 2009-12-03 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled and in production with well head properly tagged. Drilling was completed on 07/27/2009. Brine tank appeared in sound condition. Well site was seeded and mulched, but successful growth was not yet observed; therefore, the site is not yet completely restored. I observed the installation of a curlex ditch/ vegetative ditch in the area behind the well head. Noted this site was constructed with waiver approval due to distance to Stewart Run. I did not observed impact to stream, as BMPs were properly installed and functioning. No accelerated erosion or sedimentation concerns were observed in relation to well site or access road. Although it is recommended that culvert outlets are rocked.
