Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1785369 Primary Facility 2009-04-14 Routine/Complete Inspection No Violations Noted Well-site staked, not constructed. Request for waiver of distance requirement to Stewart Run. The E&S plan as compiled and drawn should minimize accelerated erosion and sedimentation and protect the stream from adverse impact, provided that the E&S plan is properly implemented and maintained.
1819794 Primary Facility 2009-08-11 Drilling/Alteration No Violations Noted At the time of inspection, well was completed to depth of 3,771 ft. Superior well performing hydro fracturing of well. Zones for fracture were 1st Bradford, lower belltarn, upper belltarn, Speechley. Well completed with 13 3/8¿ pipe to 40 ft., 9 5/8¿ pipe to 513¿ (215 sks class A), 7¿ pipe to 1054¿ (210 sks class A) and 4 ½ ¿ pipe to 3654¿(155 sks). At the time of inspection, no freeboard issues. Pit aligned with 20 mil plastic. No IW issues or E&S issues. Well sign and E&S plan located at the entrance way. E&S diagram matched site layout.
1823388 Primary Facility 2009-08-19 Routine/Complete Inspection No Violations Noted drilling complete, lined pits ok
1848361 Primary Facility 2009-12-03 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled and in production with well head properly tagged. Sign and E&S plan were also onsite. Drilling was completed on 08/03/2009. Brine tank appeared sound in condition. Well site was seeded and mulched, but successful growth was not yet observed. Noted some concerns with filter fabric fence as sections of the fence appeared unstable. It is recommended this be addressed promptly in order to prevent any possible pollution event. No accelerated erosion or sedimentation concerns were noted in relation to well site or access road. It is recommended that in order to prevent E&S concerns with access road that culvert inlets and outlets are rocked. Also noted site was constructed with waiver approval due to distance requirements to Stewart Run. I did not observed adverse impact to the stream at the time of inspection.
