1848482 Primary Facility 2009-12-03 Routine/Complete Inspection No Violations Noted At the time of inspection, the well location was staked not yet drilled. The well site was properly signed with E&S plan available for review. Site was constructed according to E&S plan. Onsite pits were dug but not yet lined. Filter fence was installed on down gradient perimeter from site. Noted that operator should monitor the structural integrity of the filter fence,due to surrounding (falling) brush that may compromise the fence, until final restoration is complete. Access road entrance and well pad were sufficiently rocked. No accelerated erosion or sedimentation issues were noted in relation to well site or access road. Observed operator plans to land farm cuttings, which is acceptable method at this site.
1852597 Primary Facility 2009-12-29 Drilling/Alteration No Violations Noted According to records, permit granted 4/17/09. Currently, PC drilling completed well to depth of 3820 ft. Well site was identified with well sign and E&S plan. No IW and E&S issues at the time of inspection. Pit aligned with 20 mil plastic and had no freeboard issues. Filter fence located topographically down gradient of site pad. Conductor pipe set at 11 ft. 7¿ pipe set at 1221 ft with 150 lead cement and 75 sks class A with 11 bbls return. No open void located. Water recorded at 270 ft. Stratigraphy consisted of shale, sandstone, and coal seams on boring log. Permit present.
1972215 Primary Facility 2011-05-09 Site Restoration No Violations Noted At the time of inspection well drilled and in production. Well tag present. Drilling Completed 12/29/2009. Site is adequately and uniformly restored with successful vegetative growth. All temporary bmp¿s have been removed from site. No accelerated erosion/sedimentation issues or industrial waste issues noted onsite. Access road stable. One brine tank located onsite atop gravel rock, observed minor dent on front of tank; although tank appears structurally sound at this time. No violations noted during this inspection.
2515036 Primary Facility 2016-08-10 Routine/Complete Inspection No Violations Noted 8/10/2016 - Produced fluids tank, annulus closed.
