1849714 Primary Facility 2009-12-03 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled but not set for production. The site was properly signed with E&S plan available for review. Access road to well site was slightly muddy and culvert pipe was damaged, it is recommended that rock be added to the access road and that culvert pipe is replaced in order to prevent erosion and sedimentation concerns promptly. Noted that pit liner did not appear to be tightly secured which could pose a threat for industrial discharge onto grounds of the commonwealth and the pit freeboard was near the 2ft limit. I observed the installation of filter fabric fence below gradient from well pad, but it is recommended the fence be maintained in order to prevent accelerated erosion and sedimentation off well pad, since areas of the fence appeared to be structurally weak. It is highly recommended that operator continue to closely monitor pit freeboard and all Bmp¿s until final restoration activities are completed to maintain compliance. A follow up inspection will be conducted by April Weiland in the near future.
