Bill Kolovich Construction Superintendent (per his request) was onsite for a review of E&S controls. The site was properly signed with E&S plan available for review. The well pad was under the final phase of construction. I noted curlex ditches (blankets) were installed around the site which lead to a sediment trap. Upon inspection of the sediment trap, I encouraged Bill to stabilize the sloped sides to prevent any erosion. No accelerated erosion or sedimentation issues were observed in relation to site or access road. BMP¿s were correctly installed and functioning properly. Drill pits were constructed, but not yet lined. Atlas plans to apply for pit solidification of drilling cuttings for this site.
1845499 Primary Facility 2009-11-24 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well sign and E&S plan located at the entrance of site. Pit aligned with 20 mil plastic. No freeboard issues. Conductor pipe set at 30 ft ¿ 20 ¿pipe used 100 sks cement. Second stringer 11 ¾¿ pipe set at 426 ft used 225 sks with 8 bbl return. Permit and sedimentary log present. Water located at 162 ft. Sedimentary consisted of ss/sh alternation and coal seam. No voids or methane detected. Filter fence set around the site. Permit set for 9000 ft well to Marcellus. First rig going to 1925 ft.

Pit aligned with 20 mil plastic. No freeboard issues. Well completed to depth. Rig removed from site. Skid of potassium chloride and two broken bags remained on-site. Called Chad Eisenman (Atlas) to clean up and remove release of chemical and repair access road at the low area where water was starting to accumulate.

Well completed to depth. Rig removed from site. Atlas hired Cal Frac to stimulate well. During Frac procedure, elevated readings of hydrogen sulfide were detected in separator according to Jim Braun (atlas). During inspection, Atlas hired SP Company to perform air monitoring. Well has been shut in and site has been secured for no admittance. SP representative was at the entrance site

Site visit performed by Jack Crooke, Mike Arch, Ed Renwick. Inspected pit, well head and separator. Also, looked at the compressor station. Well currently shut in until air results are completed.

Well completed to depth. Rig removed from site. During inspection, no field activity and Atlas still waiting on results from SP. No flow back at well.

Well completed to depth. Rig removed from site. During inspection, flow back at well was completed. Atlas was at on-site measuring separator. Pit appears to be losing water. DEP is requesting information about flow back water in pit.

Well completed to depth. Rig removed from site. During inspection, flow back at well was completed. DEP met with Jim Braunns and Jeff from Atlas. Opened get around wellhead and measured the casing pressure of 2500 psi and annulus pressure of 90 psi. Multi gas meter was laid on the outside of casing and recorded readings of 14 % combustible, 43 % CO, and 0 % H2S and 20.2 % O2. Under 78.77 (c) regulations, DEP is still requesting a monitoring plan for this well since hydrogen sulfide was detected a> 20 ppm

Well completed to depth. Rig removed from site. During inspection, flow back at well was completed. DEP met with Jim Braunns and Jeff from Atlas. Opened get around wellhead and measured the casing pressure of 2500 psi and annulus pressure of 90 psi. 1958628 Primary Facility 2011-03-08 Routine/Complete Inspection No Violations Noted At the time of inspection, well drilled and stimulated, and being monitored for H2s levels. Site Id sign and Es plan present. Well completed 3/16/10. Site re-graded (pits removed) Access road and well side stable. Site has not yet been seeded and mulched. No accelerated erosion or sedimentation issues noted. Two brine tanks onsite appear stable. All temporary bmp¿s installed & functioning. Spoke with Jeff Zarella Atlas, via telephone on 3/9/11, whom stated he thought an extension was filed for restoration of this site, but never received a response either way. Dept will further look into the matter to determine if indeed an extension was filed and or approved or denied. Operator has plans to seeded/mulch site within next week or two. At this time no violations are noted, but operator shall ensure optimal, uniform growth is achieved and site is permanently restored as a follow up inspection will be conducted in the near future.
