1819540 Primary Facility 2009-07-08 Routine/Complete Inspection Violation(s) Noted Currently, well completed on 4/9/09 to target depth of 3,933¿. Report submitted to DEP on 5/20/09. Well head completed with 7¿ pipe reduced to 4¿ with shut off valve. Three pits located on-site. According to PC, well has been fractured. Drill cutting pit was aligned with a 20 mil plastic. Water pit was approximately half filled. Frac pit was partially aligned with plastic. Plastic was torn at the seam and section had fallen into pit. Pit contained frac sand and frac water with oil sheen. At the time of the inspection, no E&S issues. 100 bbl tank located on-site. DEP called PC relative to the fracture pit. PC was going to remove the fracture pit and properly dispose the components.
1819795 Primary Facility 2009-08-11 Compliance Evaluation No Violations Noted At the time of the inspection, frac pit contents were removed from the site and frac pit area was restored to grade of the site pad. However, mud from the truck tires was noted on the roads surrounding the site. The remaining site conditions have remained the same from the previous inspection.
1838387 Primary Facility 2009-10-23 Site Restoration No Violations Noted Drilling commenced on 04/02/09. At the time of inspection, the well was properly tagged and in production. Brine tank appeared in sound condition. The area was uniformly restored with 70% vegetative cover. It is recommended that the filter fabric fence be removed. Access road was in good condition. No accelerated erosion or sedimentation issues were noted in relation to well site or access road.
