Department reviewed remediation plans submitted by Hull and associates for the remaining impoundment at this well site, and have agreed to proposed plans for "this site;" therefore, XTO had plans to reconvene restoration activities. Since last inspection, it appears water has accumulated in impoundment, presumable due to recent heavy rainfall. Based on 5/10/12 meeting operator agreed flow back was contained in impoundment, based on today's inspection department notes concerns with possible contamination of surrounding soils with "impoundment" water due to structural deficiencies in liner. Department sent Steve Smith of XTO an email addressing these concerns. Until final permanent restoration is completed well site is in violation for failure to restore (entire well site) within nine months of completion of drilling, although at this time no other violations noted.
2078527 Primary Facility 2012-06-21 Follow-up Inspection Outstanding Violations - No Viols Req'd FOLLOW UP INSPECTION to examine remediation work on "impoundments" located on well site. Department reviewed remediation plans submitted by Hull and associates for the remaining impoundment at this well site(see attached), and have agreed to proposed plans for "this site;" therefore, XTO had plans to reconvene restoration activities Since last inspection, liquid pit contents have been removed impoundment and operator currently having liner pressured washed. A tank of "clean" water was brought onsite and being utilized for pressure washing. Operator plans to examine liner integrity and mark then subsequently samples any areas that may have been penetrated. today, department could not visibly located tears or holes, but maticulious examination was not done. Again, department is recommending that operator perform soil testing on side of impoundment that was removed and hay baled, due to the possibility that pit contents may have overtopped area during rain events. Operator is actively pursing complaince status on this site.

2094013 Primary Facility 2012-08-15 Complaint Inspection In Compliance with Policy Partial Inspection- result of citizen complaint of concerns with storm water run-off. Joe Yackuboskey ICCD and OGI Rick Hoover onsite. Joe, indicated his thoughts on where and how the storm water run-off is reaching the complaintants property. Minor erosion noted access road shared to conventional well. At this time department is unable to conclusively determine that the flooding occurring on complainants property is a direct result of increased storm water runoff from Marcellus well site. A meeting is being held tomorrow, on this location with the airport director, as well as gas well operator, myself and a water quality specialist supervisor, to examine and determine if/what measure can be taken to minimize the storm water runoff that may discharge off of well site. Based on department approved ESCGP-1 it appears that well operator has contstructed and maintained bmp's as outlined in plan. Therefore, at this time no violations are being noted and operator is currenlty in compliance with current policy. Investigation is ongoing at this time.
2094022 Primary Facility 2012-08-16 Complaint Inspection Pending Inspection result of compliant. Department received a complaint of concerns with storm water runoff from well site to neighboring properties along Apache Drive. Water Quality Specialist Supervisor Edward Ritzer, Todd Hemming Indiana County Airport, and Steve Smith XTO EHS rep also present during inspection.Well sign, ES plan, and ESCGP-1 onsite. Access road appears stable. Well site adequately rocked. Ditches surrounding well pad appear to be adequately vegetated. Impoundments located on adjoining property to the well site have been "removed." Well site appears restored except form local of water impoundments, which overlapps airport project. both parties agreeded that airport will conclude final restoration per FAA regs. This is inspection is pending in order to verify LOD's as well as written confirmation of restoration plan.
2110799 Primary Facility 2012-10-16 Compliance Evaluation No Violations Noted Follow up inspection to examine restoration efforts from previously issued violations. At the time of this inspection, well site ID sign, ES plan, ESCGP-1 on located onsite. Site gated. At the time of inspection, it appears that site has been restored to PCSM. Although in like of nearby complaints operator agreed to reduce the size of the pad (rock) and replace with vegetation in order to minimize runoff from the well site, department is still waiting on final plans for pad reduction. All pits and impoundments have been removed and hydro seeded. No accelerated erosion or sedimentation issues noted at the time of this inspection with access road or well site. It appears that operator has successfully addressed previously issued violations and no new violations noted; therefore, based on today’s inspection previously issued violations for failure to restore have been corrected.
2136038 Primary Facility 2013-01-31 Routine/Partial Inspection In Compliance with Policy nspection result of request from operator for onsite meeting to examine additional restoration plans for well site. Mike Vale (Morris Knowles & Associates), Erik Greenert (XTO), Rick Hoover (OGI) Samantha Foulk (Environmental Trainee), and myself present at time of meeting. Last fall, operator restored site according to PCSM, but as a result of former complaints due to possibility of increased water runoff from the well site XTO agreed to update their PCSM to aid in minimizing the net discharge of surface waters. 2826591 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted On 1/11/19 at 1132 hrs., I conducted a routine inspection of XTO Energy, Inc.'s Indiana County Airport 8431H well (API 063-36304) in White Twp., Indiana Co. Snow cover prevented proper evaluation of site stabilization. The well pad appears to be restored and permanently stabilized. The well is properly labeled and equipped for production. A site ID sign is posted and a well tag is present at the well head. The production fluid tank is located within adequate secondary containment. There are no violations noted for this inspection.
