2503100 Primary Facility 2016-07-19 Administrative/File Review In Compliance with Policy Operator reported release.

On 7-19-16 at 15:00 Operator Rep Jason Canose reported an approximate 20 gal release of brine water. Release was discovered at 14:00 and reported to WQS C. Witham at 15:00. According to Mr. Canose the nipple on the brine tank drain line corroded and released approximately 20 gal of brine onto the ground. No waterways were affected. Mr Canose stated that the area affected by the release was approximately 3 feet by 3 feet and the brine appeared to seep straight down into the ground. He also stated that the impacted soils would be excavated on the morning of 7-20-16. A follow up inspection will be conducted at a later date.

2503113 Primary Facility 2016-07-20 Incident- Response to Accident or Event Violation(s) Noted Incident Response Inspection in response to an Operator reported release of approximately 20 gal of brine. (Inspection record #2503100).

At time of inspection well tag present. Contractors on site had just completed excavation of impacted soils. Brine tank unattached from production equipment and moved to entrance of site. Department noted corrosion on valve of brine tank. Area of release appears to be excavated to approximately 8 feet deep. Contractor stated the soil was tested with a soil probe to determine how deep to excavate. Area of excavation approximately 10 feet by 10 feet. Roll off box on site containing recently excavated soils. Department spoke with Operator Rep Jason Canose by phone to determine how release amount was calculated. Mr. Canose stated he would provide Department with that information. Department recommends Operator install temporary BMP's downgradient of site to avoid any E&S issues. Department also recommends Operator continue to monitor and maintain access road to avoid any E&S

2504871 Primary Facility 2016-07-25 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection to inspection record # 2503113 where violation issued for Operator discharge of brine on or into the grounds of the Commonwealth.

Operator Rep Jason Canose e-mailed Department with information regarding how the release amount was calculated. Operator used a spill calculator to determine that approximately 21 gal of brine was released. Previously issued violation still exists for this site.

2508305 Primary Facility 2016-08-01 Compliance Evaluation No Violations Noted Admin inspection to inspection record # 2503113 where violation issued for Operator discharge of brine on or into the grounds of the Commonwealth.

Department received 15 day response to NOV from Operator. In NOV response Operator stated release occurred due to corrosion on pipe nipple. Operator also stated that contaminated soil was removed by excavation and placed in a lined roll off dumpster for disposal. Also, the excavated area will be reseeded and mulched. Department recommends Operator install temporary BMP's as needed to avoid any E&S issues until excavated area is permanently stabilized. Operator appears to be In Compliance with Policy at this time. Previously issued violation has been corrected. A follow up inspection will be conducted at a later date. UPDATE 9-7-16: Inspection result changed from ICP to NOVIO.
