2417048 Primary Facility 2015-10-15 Administrative/File Review In Compliance with Policy Admin inspt conducted in response to inspt ID 2416988 for the 10/15 reportable release of brine water.Upon receipt of spill report, Dept contacted Opr rep R.Vrobel via phone at 1416.Mr. V stated a release of 1.5 bbl brine water w/skim of oil occurred w/in an earthen dike as a result of a leaking BT.Source of leak has not yet been identified as tank has not yet been removed and inspected.Mr. V stated that although dike is lined w/plastic, there are numerous holes in plastic.Per Mr. V, BT lost 2 in of brine & oil, equating to 1.5 bbl.Leak was discovered by well-tender at 1155 on 10/15 and subsequently reported to SWRO at 1330.Mr. V stated release appears to be visibly contained w/in dike,appears to be dripping straight down into ground.No waterways reported to be impacted.According to Mr. V, a water truck is en route to site to empty BT;Spartan will be out tomorrow (10/16) to remove tank, commence excavation.Opr appears to be ICP at this time. A FUI will be conducted to assess environmental impact and/or issue any assoc viols as necessary.

2420598 Primary Facility 2015-10-30 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspt conducted in response to viols issued under record 2417087 (enf ID 330622) for reportable release of 1.5 bbl brine water/oil.In accordance w/Spill Policy, 15 day written response received by Dept on 10/23 notifying Dept that Opr will be utilizing Alternative Remediation method for remediation of release.Upon review of 15 day written response, Opr did not identify all COCs that will be analyzed and method of attainment for remediation to be utilized for each constituent.During Dept’s on-site inspt on 10/16 (inspection 2417087), Dept was notified that a “skim of crude oil” was also released.This was not identified in 15 day written response.Dept contacted Opr rep J. Valentine via phone to request that a list be included w/15 day written response that identifies all COCs that will be analyzed and method of attainment for remediation to be utilized for each constituent.Dept recom’d that Opr also analyze COCs for crude oil in addition to brine water.Mr. Valentine agreed to re-submit 15 day written response as requested.
2434474 Primary Facility 2015-12-14 Follow-up Inspection Outstanding Violations - No Viols Req'd On-site FUI conducted in response to viols issued under record 2417087 (enf ID 330622) for reportable release of 1.5 bbl brine water/oil.At time of inspt, BT re-located w/in earthen dike, located on gravel base and plywood.Excavated contaminated soils not on-site.No accelerated E&S issues observed in relation to well site at this time.Dept recommends Opr cont’ to monitor/maintain AR as necessary in order to prevent any potential additional pollution events from occurring.Although a 15 day written response was received by the Dept on 10/23/15 notifying Dept that Opr will be utilizing Alternative Remediation method for remediation of release, a revised 15 day written response was requested by Dept on 10/30/15 (inspection ID 2420598).This revised 15 day written response has not yet been received.To date, a Site Characterization Report has not yet been received.Viols issued under record 2417087 (enf ID 330622) remain outstanding.

As the site is permanently stabilized (inspection record 2434474) and the Operator has submitted a RACR to the Department for review, violations issued under inspection record 2417087 (enf ID 330622) have been corrected. Violations may be forthcoming pending future inspections monitoring site remediation.

2454831 Primary Facility 2016-02-22 Administrative/File Review In Compliance with Policy Admin CEI conducted in response to violations issued under inspection record 2417087 (enf ID 330622) for the reportable release of 1.5 bbl brine water/oil.

Although a 15 day written response was received by the Department on 10/23/2015 notifying the Department that the Operator will be utilizing the Alternative Remediation method for the remediation of this release, a revised 15 day written response was requested by the Department on 10/30/2015 (inspection ID 2420598). This revised 15 day written response was received on 11/6/2015. The Remedial Action Completion Report was received by the Department on 12/28/2015. The ECB acknowledged receipt of the Remedial Action Plan Completion on 2/1/2016. Operator appears to be in compliance with current policy at this time.

2483690 Primary Facility 2016-05-02 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to violations issued under inspection record 2417087 (enf ID 330622) for the reportable release of 1.5 bbl brine water/oil. Violations were previously corrected under inspection ID 2440205.

Although a 15 day written response was received by the Department on 10/23/2015 notifying the Department that the Operator will be utilizing the Alternative Remediation method for the remediation of this release, a revised 15 day written response was requested by the Department on 10/30/2015 (inspection ID 2420598). This revised 15 day written response was received on 11/6/2015.

The Remedial Action Completion Report was received by the Department on 12/28/2015.
