1839292 Primary Facility 2009-10-23 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was properly tagged and in production but, I observed a pump jack leak. I immediately phoned inspector Paul Wyrwas who subsequently notified Chris Shea of Keyrock, who assured Paul the problem would be addressed immediately, within the hour. I observed minor offsite impact (puddle of fluids with negligible discoloration of vegetation) from the leak. It was also recommended that the access road be maintained as various ruts were beginning to form.
1839301 Primary Facility 2009-10-27 Follow-up Inspection No Violations Noted This inspection was a follow up to see if a violation (10-23-09, pump jack leak) was corrected prior to any NOV being issued, as operator was notified immediately of problem, and was to address issue at once. After notifying Chris Shea of the leak, he notified myself and Paul Wyrwas that the problem was addressed and clean up procedures took place on 10-23-09 at 1220pm. At the time of this inspection, the pump jack was fixed and any offsite fluid release was cleaned up. It did not appear that the fluid release traveled offsite more than initial inspection and all preventative pollution efforts and maintenance techniques were properly and rapidly executed to control/repair leak. Paul Wyrwas was also to follow up with an inspection of well.
