According to records, drilling occurred 11/11/1999-1/17/1999. Well label present on brine tank. Department recommends Operator install well tag on well in a legible, visible, and permanent manner as per current regulations. Brine tank appears sound, located on plywood.
2455258 Primary Facility 2016-02-29 Administrative/File Review In Compliance with Policy SPILL.

On 2/29/16 at 1645, Operator representative Scott Bowers reported a release of 23 bbl rainwater/brine water mixture to the ground to SWRO DO April Weiland. The spill was discovered on 2/29/16 at 1610 and is reported to have resulted from a valve failure on the brine tank. Per Mr. Bowers, a temporary plug was placed in the tank valve. A cleanup crew will be on-site tomorrow (3/1) to assess the environmental impact and collect samples in accordance with the Spill Policy. Per Mr. Bowers, it is unknown when the release occurred. The well is checked monthly. The quantity lost is based on tank readings. No waterways are reported to have been impacted. According to Mr. Bower, the tank is currently unable to be moved due to a “frozen ice cube” inside the tank.

2455298 Primary Facility 2016-03-01 Administrative/File Review In Compliance with Policy Admin inspt conducted as a follow-up to inspection ID 2455258 for 2/29 reportable release of 23 bbl rainwater/brine water mixture to ground.Upon receipt of spill notification,Dept contacted Opr rep Scott Bowers via phone at 0838.According to Mr. B, rainwater accumulated in BT when top of tank settled causing water to trickle into tank.Water then got into valve,froze,broke,resulted in 23 bbl of water trickling out onto ground until remaining water in tank froze into “massive ice cube”.Damaged valve was removed and BT plugged.Mr. B believes fluids released were fresh water because there is no dead veg and well does not produce brine water,but believes,however,that there could have been some brine.Dept recommended that, if possible, samples of fluids released are collected in order to determine nature of the water.Mr. B believes that last time well was checked would have been on or about 1/29/16, therefore it is believed the release occurred between this date and when it was discovered on 2/29.ADD'L COMMENTS ON
2461784 Primary Facility 2016-03-01 Incident- Response to Accident or Event Violation(s) Noted On-site inspt conducted in response to inspection ID 2455258 for 2/29 reportable release of 23 bbl rainwater/brine water mixture to ground.At time of inspt, Scott Bowers (Production Foreman) and Spartan on-site.Spartan currently excavating soils where fluids were released.BT has not yet been moved due to a “massive ice cube” w/in BT, the top of which is estimated to be located at the 70 bbl line mark on BT.BT valve is plugged.Mr. B believes that a release of only freshwater occurred at this location as this well typically does not produce brine water.As a release of a brine/rain water mixture was reported,Dept recommends Opr collect samples and perform remediation as necessary in accordance w/Dept’s Spill Policy.Dept recommends Opr cover area of excavation w/plastic and/or tarp in order to prevent potential further spread of contaminants should a precipitation event occur. Violations are being issued for this reportable release of production fluids.
2465319 Primary Facility 2016-03-10 Compliance Evaluation In Compliance with Policy Admin CEI conducted in response to viols issued under inspection 2461784 (enf ID 340507) for 2/29/16 reported release of 23 bbl brine water/rainwater mixture to the ground.In accordance w/Spill Policy,15 day initial written report received on 3/10/16 notifying Dept that Opr will be utilizing Alternative Remediation Method for remediation of this release.Opr will be analyzing the following constituents: Aluminum, Barium, Boron, Iron, Lithium, Manganese, Selenium, Strontium, Vanadium, Zinc, and Chloride.Opr appears to be ICP at this time insofar as the Opr has identified their intent and schedule for remediation in accordance w/Dept’s Spill Policy.Viols issued under record 2461784 (enf ID 340507) for this reported release have been closed. Violations may be forthcoming pending future inspections monitoring site remediation and permanent stabilization.
2470982 Primary Facility 2016-04-04 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to violations issued under inspection record 2461784 (enf ID 340507) for the 2/29/16 reported release of 23 bbl brine water/rainwater mixture to the ground.

The requested NOV written response was received by the Department on 4/4/2016 and has not yet been reviewed. Violations issued under inspection record 2461784 (enf ID 340507) were previously closed under inspection ID 2465319.
