While on-site, the Department notified Operator representative Brian of the below-noted observations. Brian stated that personnel would be sent to the location to investigate and will then follow-up with the Department.

The Department is requesting the following information be provided with the NOV response:

1) The source/cause and composition of the fluids released 2) The quantity released 3) The last time a well tender was at the location

The Department recommends the operator remediate this release in accordance with the Department's Spill Policy.

2626450 Primary Facility 2017-08-09 Administrative/File Review In Compliance with Policy Admin inspection in response to violations issued under inspection record 2623475 (enf ID 357233) for failure to properly control and dispose of residual fluids.

Operator representative Brian Stanford (Production Manager) contacted the Department via phone to provide an update.

Per Mr. Stanford: Soil was placed over the impacted soils and seeded. The fluid released was soap suds. Brine may have been mixed in with the soap suds. Only a minimal amount of fluids were released, estimated to be one gallon. The release resulted when the well was blown. The date this occurred is unknown at this time.

The Department stated that the impacted soils had a petroleum odor during the Department's inspection. Mr. Stanford does not believe oil was released because this well does not produce oil and perhaps the odor was that of brine.

The Department electronically provided operator representative Brian Stanford with a copy of the Department's Spill Policy (Document Number: 800-5000-001).

The requested NOV response was submitted to the Department via e-mail by operator representative Brian Stanford.

2639413 Primary Facility 2017-09-25 Compliance Evaluation No Violations Noted Compliance evaluation inspection in response to violations issued under inspection record 2623475 (enf ID 357233) for failure to properly control and dispose of residual fluids.

At time of inspection, area of prior release appears to have been remediated and is exhibiting 70% uniform and adequate vegetative growth. The Department recommends the operator continue to monitor and maintain this area in order to ensure permanent stabilization is maintained. Previously issued violations have been corrected. No new violations have been noted at this time.

Disposal records/manifests of contaminated materials not yet received. Additional violations may be forthcoming pending the receipt and review of these documents.
