2707924 Primary Facility 2018-03-21 Administrative/File Review In Compliance with Policy Operated reported a release to the ground on 3/20/2018 at 5:32pm. The Operator indicated that approximately 34 gallons of crude oil was released to the ground at the well site and clean had begun. A follow up phone call to the Operator was placed on 3/21/2018. The Operator indicated that the release occurred after the on site tank was repaired. The Operator indicated that the flange was replaced and in the process of pumping the produced fluid back into the tank a hose fitting failed, resulting in a release to the ground. The Operator indicated that the released fluid did not reach the waters if the Commonwealth. The Operator indicated that the impacted area has been visually removed and placed in steel drums. The Department will follow up with an on site visual inspection. The Operator is in compliance with the current spill policy.
2708820 Primary Facility 2018-03-22 Incident- Response to Accident or Event Violation(s) Noted Incident response inspection to inspection record #2707924 where operator reported a release of approximately 34 gallons of crude oil. At time of inspection site snow-covered (photo #1). Two 42 gal barrels on site. Brine tank located in metal secondary containment unit. Department contacted operator rep Curt Elkin by phone from site for update. Left message. Department contacted Mr. Elkin at 13:58. Mr. Elkin stated that a hose fitting failed causing oil to spray onto the snow. Mats/pads were placed on the ground to absorb the oil. These mats/pads and associated snow were placed in two bbls and are awaiting removal. No soil excavation was conducted.

Due to snow cover, Department unable to determine if remediation efforts appear adequate. A follow up inspection will be conducted after snow-melt. A violation is being issued for operator discharge of brine or other fluids.

Department received operator response to NOV (photo #1). Violation issued on 3-22-18 remains outstanding.

At time of inspection bbls observed in above inspection containing contaminated materials have been removed. Site vegetated. No black residue or possible contaminated soil observed at this time. Department contacted operator rep Lisa Raffle to request disposal manifests. Ms. Raffle stated she would follow up with Department with that information. Violation issued on 3-22-18 remains outstanding. A follow up inspection will be conducted at a later date.

2719791 Primary Facility 2018-04-18 Compliance Evaluation No Violations Noted ADMIN inspection to inspection record #2708820 where a violation was issued for operator discharge of brine or other fluids.

Department returned operator rep Lisa Raffle's phone call. Ms. Raffle stated that oil and snow were placed in bbls on the day of the release. Absorbent pads were used to soak up any residual oil that may have been missed. The bbls were emptied into the brine tank on site. The absorbent pads were disposed of by Safety-Kleen. Operator appears to be in compliance at this time. Violation issued on 3-22-18 has been corrected and is closed.
