The Department observed a hissing sound and observed bubbling from the well head. A gas odor was also observed. This concern is being forwarded to the OGI for follow-up.

At 1246, the Department contacted operator representative Jenny Reefer by phone. Per Ms. Reefer: The quantity released was determined by tank readings. The tank has been removed and may not be replaced. All visibly impacted soils have been removed and the area is temporarily stabilized.

The Department requests that the waste manifests/disposal records are submitted to the attention of Samantha Foulk at the provided address upon disposal of all contaminated materials excavated in response to this release.

2702273 Primary Facility 2018-02-20 Administrative/File Review Outstanding Violations - Viols Req'd Admin inspection conducted in response to violations issued under inspection record 2646512 (enf ID 358478) for failure to properly control and dispose of production fluids.

The Department contacted operator representative Jenny Reefer by phone to determine if the Department's Spill Policy previously provided to the operator (inspection record 2658263) was implemented at the location of the release. Ms. Reefer confirmed receipt of the Department's Spill Policy and stated that the tank was removed, lime and seed were spread on the site, and grass is growing; no contaminated soils were removed. The Department recommended that the contaminated soils are removed and disposed of properly in accordance with the Department's Spill Policy. Ms. Reefer stated that she does not understand why the soils need to be dug out if grass is growing as it will cause additional unnecessary earth disturbance. 2710125 Primary Facility 2018-03-23 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to violations issued under inspection record 2646512 (enf ID 358478) for failure to properly control and dispose of production fluids.

The Department (WQS S. Foulk and WQSS A. Weiland) contacted operator representative Jenny Reefer by phone as a follow-up to inspection record 2702273. The Department reviewed the Department's Spill Policy (Document Number: 800-5000-001) with Ms. Reefer. The Department recommended that all visibly impacted materials are removed and disposed of properly in accordance with the Spill Policy in order to prevent any further pollution to the ground and/or prevent any potential pollution events from occurring in Waters of the Commonwealth. The Department recommended utilizing soil conductivity readings, both background readings and readings in the area of impact, to aid in delineating the release and to ensure all contaminated soils are removed. Ms. Reefer stated that they would try to remove the soils at the site within the next couple of week

2728957 Primary Facility 2018-04-20 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to violations issued under inspection record 2646512 (enf ID 358478) for failure to properly control and dispose of production fluids.

The Department inquired via email if the excavated soils have yet been excavated. Operator representative Jenny Reefer indicated yes and that the operator is currently awaiting a soil sample to see if WM will accept.

2748679 Primary Facility 2018-07-03 Compliance Evaluation No Violations Noted CEI to inspection record #2646512 where violations were issued for failure to collect brine and other fluids and failure to control and dispose of fluids in a manner that prevents pollution of the waters of the Commonwealth.

At time of inspection well tag present. Brine tank removed from site and that area appears to be seeded and mulched. Vegetation in that area is growing but does not yet appear to have reached >70 % uniform growth (photo #1). Department recommends operator re-seed this area to ensure site is permanently stabilized. Area down-gradient of where brine tank was located is vegetated (photo #2). The Department did not observe the diversion ditch referenced in inspection record #2658259. This area up-gradient of the site is vegetated. Excavated soil appears to have been removed from site. As of this date Department has not received disposal manifest from operator.
