2619169 Primary Facility 2017-06-22 Administrative/File Review In Compliance with Policy Admin inspection in response to violations issued under inspection record 2592206 (enf ID 353324) for failure to have an E&S plan on-site during all stages of an earth disturbance activity.

After review of the NOV response, the Department contacted operator representative Marc Jacobs by phone. The Department requested either an E&Splan or documentation demonstrating and verifying that the earth disturbance related solely to the plugging of the well was less than 5,000 square feet be submitted to the Department. Mr. Jacobs agreed to the Department's request. The Department also requested that written consent from the landowner be submitted verifying that the production equipment is to remain on-site. Mr. Jacobs agreed to the Department's request.
