2627654 Primary Facility 2017-08-15 Routine/Complete Inspection In Compliance with Policy At time of inspection plugging rig on site and actively removing pipe from well bore. Department observed what appears to be black water spraying/leaking from well head when contractors removed each section of pipe. It appears most of this water is being directed into a lined pit. Operator rep James Krepps stated that hay was being brought to site to help contain the remaining water. Department recommends operator contain fluids that do not reach the pit and dispose of any soils that have been negatively impacted by the plugging of this well in a manner approved by law. A follow up inspection will be conducted at a later date.

2629716 Primary Facility 2017-08-25 Routine/Complete Inspection No Violations Noted Follow up inspection to inspection record #2627654 where Department observed black water being released from piping during active plugging operation.

At time of inspection, well appears to be plugged. Plugging rig not on site. No roll off boxes on site. Operator stated in inspection record #2627654 that hay would be used to contain fluids spraying from pipe string. At the time of this inspection, hay has been placed on site around cellar and surrounding area. No black staining observed.

2789729 Primary Facility 2018-10-16 Site Restoration No Violations Noted Inspection conducted in response to a Well Site Restoration Report received by Department. At time of inspection site and access road appear permanently stabilized and restored.

This restoration report is approved.
