2709509 Primary Facility 2018-03-15 Preoperation Inspection No Violations Noted On March 15, 2018, the Department performed an inspection of the EQT/Rice Energy Beta Cameron well site. Present at time of inspection was Rex Nelson (EQT). The site is located in Richhill Twp., Greene Co. The inspection was related to a request for a pre-operational inspection of the site before the operator commences drill operations.

The operator was stoning the well pad at time of inspection in preparation to begin drilling operations on the site. The operator was in process of stabilizing the site with seed and mulch. Construction of the PCSM structures has begun and the area is being temporarily stabilized.

6 wells have been permitted for the but none have been drilled at time of inspection. The inspection was a pre-spud inspection for the 6 permitted wells.

2710921 Primary Facility 2018-03-27 Drilling/Alteration No Violations Noted The well was spud on 3/16/18. Operator drilled a 30-inch borehole to 40-feet. A 26-inch conductor pipe has been set at 40-feet and cemented through tubing to the surface. A well cellar has been installed around the conductor pipe.

The operator drilled a 20-inch borehole adjacent to the well cellar and set and cemented a 16-inch mouse hole pipe to the surface.

2717498 Primary Facility 2018-04-12 Drilling/Alteration No Violations Noted The operator has set and cemented surface and coal protective casings. Drilling intermediate borehole section @ 2,219'.

Present at time of inspection was Tommy Lawson (Drilling consultant). The site is located in Richhill Twp., Greene Co. The inspection was related to a reported spill of oil based drilling mud by the operator.

The operator reported a approximate 2 barrel oil based drill mud release to the well pad on 5/27/18. During rig setup to begin drilling operations on a new well on the site, gas was vented through the flare stack, purging the flare stack of residual oil based drill mud in the lines. Approximately 2 barrels were released to the site with 1 1/2 barrels being discharged off containment to the well pad and well pad berm on the southwest corner of the well pad. The vegetation on the well pad berm and cut slope is stressed at the area of the release.

2770262 Primary Facility 2018-08-28 Compliance Evaluation No Violations Noted On August 28, 2018, the Department performed an inspection of the EQT/Rice Energy Beta Cameron well site. Present at time of inspection was Jeff Dunesing (Completions consultant). The site is located in Richhill Twp., Greene Co. The inspection was related to a reported spill of oil based drilling mud by the operator, on 5/26/18, and violations issued on 5/30/18 by the Department.

The operator reported an approximate 2 barrel oil based drill mud release to the well pad on 5/26/18. During rig setup to begin drilling operations on a new well on the site, gas was vented through the flare stack, purging the flare stack of residual oil based drill mud in the lines. Approximately 2 barrels were released to the site with 1 1/2 barrels being discharged off containment to the well pad and well pad berm on the southwest corner of the well pad. The vegetation on the well pad berm and cut slope was stressed at the area of the release, during the 5/30/18 inspection.

2803853 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted On October 22, 2018, the Department performed an inspection of the EQT/Rice Energy Beta Cameron well site. Present at time of inspection was Jamie Davis (EQT). The site is located in Richhill Twp., Greene Co. The inspection was related to a request by the operator to meet on site and review some proposed minor modifications to the permit for repairs to the PCSMs on site.

The operator has requested for some minor modifications to the ESCGP2 permit to place rock aprons in the cut slopes of a wet pond and rain garden which failed (PCSMs). The operator has also requested to extend rock aprons from culvert discharges down the fill slope to the PCSMs. The minor modifications were reviewed and approved by the Department in the field.

3 wells (8,10, and 12) have been completed and are equipped for production on the well pad.

3 wells (2,4, and 6) have been drilled to TD, but have not been hydraulically fractured.
