2773798 Primary Facility 2018-09-10 Complaint Inspection No Violations Noted On September 10, 2018, the Department performed an inspection of the EQT/Rice Energy Magnum well site. Present at time of inspection was Mike Jarrett (EQT). The site is located in Center Twp., Greene Co. The inspection was routine inspection during drilling operations related to a complaint.

The operator was continuing drilling operations on the Magnum 7 (956497) well with the Precision 539 drill rig. The secondary containment on site was in good condition.

2843502 Primary Facility 2019-02-21 Incident- Response to Accident or Event Violation(s) Noted On Tuesday February 19, 2019 I received notification of a produced water release at the Magnum Well Site located in Center Township, Greene County. The release had occurred and was reported to the Department on February 17, 2019. On Tuesday morning I spoke on the telephone with Mr. Corey Giles from EQT Production Company (EQT) regarding details of the incident and the remediation. On February 19 and February 21, I. conducted incident inspections to further assess the situation. The February 21 inspection was conducted after receiving additional requested details from Mr. Giles.

On February 17, EQT representatives discovered the release which resulted from a dump valve wash-out. It was estimated that approximately 25-30 gallons was released from the unit as a result of the wash-out. This occurred on the "B" side of the unit and impacted the well pad in front, along side and behind the unit. EQT's remediation contractor, SPSI, excavated the impacted materials for off-site disposal and backfilled the ex
