Present at ime of inspection was Jon Bechdel and Aaron Parker (Rice). The site is located in Jackson Twp., Greene Co. The inspection was related to a frac fluid release reported by the operator.

The operator reported a release of frac fluid to the well pad with approximately 3 barrels being released to the well pad near the entrance to the well pad from the access road. The operator reported that during completion operations of the 4H well on site a transducer malfunctioned causing the release valve on the 4H well to release pressure and frac fluid to the well pad. The frac fluid was released in a "geyser" manner which covered the well pad secondary containment and approximately 3 barrels were released to the well pad. The operator had SPSI on site excavating impacted stone from the well pad and storing it on site in a rolloff box for disposal. The remediation was being directed by Envirotrac. En

Present at ime of inspection was Joe Fletcher (EQT). The site is located in Jackson Twp., Greene Co. The inspection was related to a frac fluid release reported by the operator on 9/28/17, and noted violations.

On 9/28/17, the operator reported a release of frac fluid to the well pad with approximately 3 barrels being released to the well pad near the entrance to the well pad from the access road. The operator reported that during completion operations of the 4H well on site a transducer malfunctioned causing the release valve on the 4H well to release pressure and frac fluid to the well pad. The site is located in Jackson Twp., Greene Co. The inspection was CEI related to violations noted on 9/28/17 and 12/7/17.

The operator has finished flow back operations on the recently hydraulically fractured wells on the site. The flow back equipment has been removed. The operator has remediated the two reported frac fluid releases on the well pad. The operator has recently completed construction of the PCSM structures on the site.

4 wells have been drilled to TD on the site, and have been equipped for production.
