On 05/22/17 at approximately 0900 hours I conducted a routine complete inspection of the site. The Act 9 sign was in place and all required documents were present at the site. ECA submitted a restoration extension request dated October 6th, 2015 for the SGL 223 well site. On February 29th, 2016, the extension request was approved and the new date for restoration was March 1st, 2017. On 03/08/17 ECA submitted an additional Well Site Restoration Extension Request. A Well Site Restoration Extension Request can be granted once for a period of up to 24 months. The new restoration request is denied, since one had previously been approved.

The site was to be restored by 03/01/17. As of 05/22/17 the site was not restored. Two modular aboveground storage tanks were present at the site at the time of inspection. Please note that some of the below violations are not listed and denoted for a violation in the above section, as there is no selection for them.
