2474697 Primary Facility 2016-04-13 Compliance Evaluation No Violations Noted On 4/13/16, I performed a compliance evaluation inspection at the GH46 in relation to a spill from 12/29/15 and made the following observations:

•All required documents and signage were in place at the site. •Multiple bare areas of soil as well as areas of patchy or sparse vegetation were noted but did not appear to be related to the spill. No violation. •All 12 permitted wells were present and active at the time of inspection. No violation. •Many barrels of desiccant were found on site with no containment. No violation, containment is recommended. •Standing water in the vicinity of the related spill tested at 851¿s. No violation. Conclusion: •Initial remediation efforts detailed in the 15 day response appear to have been effective, a site characterization report is still required within 180 days of the spill date and further remediation may be required.

2489025 Primary Facility 2016-05-20 Incident- Response to Accident or Event In Compliance with Policy On Thursday, May 19, 2016, a brine spill had occurred at the GH 46 Well Site located in Center Township, Greene County. The release occurred at an above ground storage tank (AST) that is connected to a series of salt dryers. A hose placed through a port at the top of the AST, to off-load material, continued to syphon brine from the tank when the hose was disconnected from the truck. It was estimated that approximately 60-70 barrels of brine was syphoned from the AST and into containment surrounding the tank. The containment was vacuumed out with a truck and the contents were placed back into the AST.

The release was completely contained and there was no impact to water or the ground surface. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.

2493664 Primary Facility 2016-06-03 Incident- Response to Accident or Event In Compliance with Policy On Friday, June 3, 2016, I was notified of a spill that had occurred at the GH 46 Well Site located in Center Township, Greene County. Salt from the drying units is placed into 55-gallon drums after reaching its effective capacity. The drums are staged on-site inside of secondary containment prior to off-site disposal. Rain water from multiple precipitation events had mixed with salt residue on the drums. The salt went back into solution and into the containment structure. Given the amount of precipitation at the time, the fluid inside containment overflowed the berm and stained the gravel. The stained gravel was scraped from the pad and disposed off-site. Soil conductivity readings were taken to determine the extent of excavation and confirmation samples were collected for analysis following the excavation. CNX Gas Company has since made arrangements to have the drums removed from the site and staged with their environmental contractor at a facility prior to disposal.

2493817 Primary Facility 2016-06-03 Incident- Response to Accident or Event In Compliance with Policy A mist from a blowdown tank was released through a valve which impacted the well pad. It was estimated that approximately three gallons of product was inadvertently released through the valve which impacted an area of the well pad approximately 40 feet by 40 feet. The impacted gravel was scraped from the surface of the well pad for off-site disposal.
2573118 Primary Facility 2017-03-20 Routine/Complete Inspection No Violations Noted On Monday, March 20, 2017, I conducted a routine complete inspection of the GH-46 Gas Well Site located in Center Township, Greene County.

The Act 9 sign was clearly legible at the site entrance along Jacobs Run Road. A sign depicting gas well API numbers was also clearly posted at the site entrance and all required documentation was available on-site. The access road and the well pad were in good condition. The operator should continue their periodic monitoring program and perform all necessary maintenance to the access road and well pad upon identification.

General site construction and PCSM controls were inspected and confirmed to have been completed according to the permitted site plans. The site is completely vegetated. Both filter sock and silt fence were observed in place in several areas surrounding the site and along the access road. The Filter sock and silt fence were also observed to require maintenance. The operator should evaluate site vegetation and whether the filter sock and silt fen
