2696366 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted On Wednesday, February 14, 2018, I conducted a routine-complete inspection at the Charles Nelson No. 3 Well Site. The Charles Nelson No. 3 is a horizontal oil well located in Center Township, Greene County.

The access road and the well pad were in good condition and the site has been stabilized with vegetation. A site specific Emergency Response Plan was also available on-site.

The earthen berm secondary containment structure around the production tanks was observed to be partially vegetated and in good condition. Gravel lined the ground surface within the earthen berms. There were no signs observed of fluids having been inadvertently released prior to inspection. The production tanks were observed to be in good condition and appeared to have been recently painted.

The oil well and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.

2834074 Primary Facility 2019-01-22 Routine/Complete Inspection No Violations Noted On Tuesday January 22, 2019 I conducted a routine-complete inspection at the Nelson 3H Horizontal Oil Well Located in Center Township, Greene County.

The access road and the well pad were observed to be in good condition.

The site has been stabilized with vegetation as observed through a thin to sparse cover of snow.

The earthen berm secondary containment around the production tanks appeared to be in good condition and was observed to be vegetated. The production tanks also appeared to be in good condition.

Although the operator is in compliance with current regulations, they should consider using secondary containment constructed of impermeable materials. A release of fluid within the berm can result in a violation being issued to the operator.

The oil well and the associated infrastructure appeared to be in good condition and functioning properly. The well was tagged with identification.

No violations were identified during the inspection.
