2569062 Primary Facility 2017-03-08 Routine/Complete Inspection No Violations Noted On Monday, March 6, 2017, I conducted a routine/complete inspection at the J. Moninger OG Well Pad located in Morris Township, Greene County.

The Act 9 Sign was in place and clearly legible at the site entrance, as were the gas well API identification signs below the Act 9 signs. All required documentation was also available on-site.

The access road and well pad were in good condition. Some minor rill formation was observed on the access road. The operator should continue their periodic monitoring program and conduct maintenance as warranted.

All PCSM controls have been installed according to plans and are functioning as intended. The site is stabilized via a combination of lush vegetative growth, seed and erosion control matting. The most recent seeded areas exhibit some vegetative growth. The operator should continue their periodic monitoring program and mitigate any signs of accelerated erosion and sediment movement upon identification.

2589852 Primary Facility 2017-05-03 Site Restoration No Violations Noted On Wednesday, May 3, 2017, I conducted a restoration inspection at the J Moninger OG Well Site located in Morris Township, Greene County. The inspection was the result of an operator submittal of Well Site Restoration Reports in accordance with 25 Pa. Code Section 78.65(3). The well site restoration is an interim restoration while the gas wells on the pad remain in the production stage. The inspection was conducted to confirm that the interim restoration was completed in accordance with the operator's site restoration plan.

Inspection of the site confirmed that the site has been restored to the configuration depicted on the restoration plans.

No violations were identified during the inspection.
