Present at time of inspection was Walter Nelson and Jamie Davis (EQT). The site is located in Jackson Twp., Greene Co. The inspection was related to a request by the operator to meet on site and review a proposed minor modifications to the permit, and review the site for termination of the ESCGP2 permit.

The operator has requested for some minor modifications to the ESCGP2 permit for the installation of two rock aprons on the cut slopes of the PCSM construction. The operator has also requested to install a cut slope drain which will extend to the PCSM collection channel. The minor modifications were reviewed and approved by the Department in the field.

Construction of the PCSM structures on site has been completed. The vegetation growth on the area is poor and does not meet the required greater than 70% uniform, perennial cover. The operator has reclaimed a parking area along the access road near the w
