2463068 Primary Facility 2016-03-04 Administrative/File Review In Compliance with Policy On Thursday, March 3, 2016, a GPU unit for well MOR9BSH released approximately three gallons of glycol due to an equipment malfunction. An estimated one gallon was retained on the concrete pad and the remaining two gallons spilled off of the pad and onto the gravel. Absorbent pads were used to contain and remove glycol from the surface of the gravel and concrete. The impacted gravel was then scraped up and removed for disposal. The malfunctioning equipment was repaired.

The incident was reported and corrective measures enacted upon identification. No impacts to water occurred and the affected area was remediated. The matter has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.

2683689 Primary Facility 2018-01-12 Routine/Complete Inspection No Violations Noted On Friday, January 12, 2018, I conducted a routine-complete inspection at the MOR 9 Well Site located in Morris Township, Greene County.

The gas well, DSH (059-25472), and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled as required.

The well cellar associated with well DSH (059-25472) was observed to be filled with water with bubbles rising from below. CNX Resources (CNX) was immediately notified of the observations via email with photos (Department supervisors cc'd) and telephone. I was informed that the bubbles were suspected to be from an annulus vent tube and that personnel from production were being notified to look into the situation.

No violations were identified during the inspection.
