1897060 Primary Facility 2010-07-09 Routine/Complete Inspection No Violations Noted The operator submitted a request for a six month site restoration extension on 4/7/10 due to pipeline and frac equipment scheduling difficulties. The well reached total depth on 10/18/09 and the ID sign and E&S plan was on site. All E&S controls were installed properly and need to be maintained and inspected after every significant rainfall event. The following extension has been approved, and the deadline for the site restoration is 1/18/11. The Department requests that the fill slop be inspected on a regular basis to hinder any accelerated erosion along with weekly inspections and inspections that follow any significant precipitation that might cause a runoff event.
