1775858 Primary Facility 2009-03-10 Routine/Complete Inspection No Violations Noted A waiver inspection was conducted. The earth disturbance will be < 100 FT from Muddy Creek. Well site is constructed and operator proposes to expand site. Although the waiver is for the South Branch, the proposed well site has a stream on either side. Muddy Creek and the South Branch Muddy Creek join. Both streams are close to the distance resriction. The operator should be conscious of both streams and utilize BMP¿s during earthwork activities. Proposed BMP's should protect stream from adverse impact if properly installed and maintained. The waiver and E/S Plan were reviewed and approved. A ESCGP-1 was submitted to Greene County Conservation District.

1884713 Primary Facility 2010-04-22 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well. The Operator is currently drilling during this inspection. The E/S controls appear to be in order. The site is in good condition. The Department approved a waiver for this site for the 100 ft distance restriction. The Operator is drilling on a closed-loop system and has no drilling pits. Any spills should be immediately cleaned-up and reported to the Department.
1887116 Primary Facility 2010-05-18 Routine/Complete Inspection No Violations Noted The Department was at the above referenced Marcellus Well to conduct a routine inspection. The Operator is currently drilling on site with a closed loop drilling system. There are no drilling pits on site at this time. The E/S controls are in order. There is a significant amount of water laying around the site and should be pumped out as a pollution prevention measure. If a spill occurs, the contaminant will get into the water on site. The Operator can recycle this water to use on another site. The permit numnber is marked wrong on the site ID. The permit number is 059-25138 and not 059-25137.

On 02/01/17 at approximately 1030 hours I conducted a site restoration inspection of the Fuller A pad in response to an NOT submittal by Energy Corporation of America. The Act 9 sign was in place and all required documents were present at the site. The site was well vegetated and no signs of erosion were observed. The site meets the requirements for permit termination. The NOT is approved.
