2677591 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted On Wednesday, January 3, 2017, I conducted a routine-complete inspection of the Viola Cole Estate #1 well (059-25061) located in Washington Township, Greene County. The inspection was conducted after receiving the Well Site Restoration Report for the site.

The area was snow covered at the time of inspection although the access road and the well pad appeared to be solid, level and in good condition. Site vegetation could not be completely assessed due to the current snow cover although a significant amount of vegetation was observed extending above the snow. The earthen berm containment around one poly tank and one metal tank appeared to be in good condition and was vegetated.

The gas well appeared to be in good working order, as was the associated piping and infrastructure. The well was not permanently labelled with the associated well permit number. The Oil and Gas Act, 58 Pa. C.S., Section 3211(h), requires that each well be permanently labelled with the associated permit number. The operator shoul
