2659998 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted On Tuesday, November 14, 2017, I conducted a routine-complete inspection at the GH21 gas well located in Center Township, Greene County.

The gas well appeared to be in good working order and the well was labelled as required. The poly production tanks are staged within an earthen berm containment structure which is partially vegetated.

No violations were identified during the inspection.

2692807 Primary Facility 2018-02-09 Routine/Complete Inspection No Violations Noted On Friday, February 9, 2018, I conducted a routine-complete inspection at the Tustin GH21 Well Site located in Center Township, Greene County.

The access road and the pad were in good condition. The area was partially snow covered and vegetative growth was observed through the snow cover and in areas without snow.

The earthen berm secondary containment around the poly production tanks was partially vegetated and no signs of a release were apparent. Frozen water was observed inside the berm.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.

2836869 Primary Facility 2019-01-31 Routine/Complete Inspection No Violations Noted On Thursday January 31, 2019 I conducted a routine-complete inspection at the GH21 CBM Well Site located in Center Township, Greene County.

Access to the well is partially shared with the GH19CV vertical gas well site. The access road and the well pad area were in good condition.

Uniform vegetative growth was observed through sparse snow cover.

The earthen berm secondary containment structure around the production tanks appeared to be in good condition and vegetative growth was observed through sparse snow cover. An area of thinner vegetative growth was observed in the area where trucks off-load fluid from the production tanks. The operator should consider reseeding this area to further stabilize the berm structure. The production tanks appeared to be in sound condition.

Although the operator is compliant with current regulations, they should consider using secondary containment constructed of impermeable materials. A release of fluid within the berm can result in a violation being issued to the ope
