2768491 Primary Facility 2018-08-23 Routine/Partial Inspection No Violations Noted On 08/23/18 at approximately 1015 hours I met with Curtis Small of Chevron to discuss possible redline changes. The operator of an adjacent conventional well accesses their well from the Siegel well pad entrance. The conventional well operator had a preexisting access road that was modified and graveled in accordance with the approved E&S plan during construction of the Siegel well site. The conventional operator indicated to Chevron the access road as modified is steeper than it was originally. Chevron proposed reducing the grade of the road. Redline approval was granted to reduce the grade of the access road within Chevron's LOD.
2846017 Primary Facility 2019-02-15 Compliance Evaluation No Violations Noted On 10/13/18 a synthetic based mud spill occurred at the site, resulting in the issuance of an NOV.

On 02/15/18 I conducted a compliance evaluation inspection at the site. The impacted pad slope, channel, and stormwater basin had been remediated. The remediated areas were properly stabilized and there were no signs of erosion observed. The violations have been corrected and there were no violations observed at the time of inspection.

2853083 Primary Facility 2019-03-13 Drilling/Alteration No Violations Noted This well has had the top hole drilled and cased and is tagged and inside a protective cattle gate waiting on the fluid Rig to arrive and finish drilling. There was no Drill Site Supervisor present on site at the time of this inspection. All equipment staged on site looked ok.

NOTE: I contacted Chevron and had a Compliance Specialist come to the site to darken the well sign information in a bit more clearly as the recent heavy rain made it a bit difficult to read.
