2524778 Primary Facility 2016-09-15 Routine/Complete Inspection Adverse Events or Actions Reported Met w/ Rick Lewis and landowner at residence to inspect cavernous area that has developed around an old plugged(?) or abandoned well beside garage. The void formed ~one foot below the lawn surface in a ~3 foot radius surrounding the old well, and is 4 feet deep. The well consists of an 18 inch diameter concrete cylinder-shaped plug, ~18 inches high, ~2 feet below surface. This plug has the imprint of what appears to have been the old wooden conductor on its sides. The plug sits on a much smaller concrete cylinder, possibly ~6 inches diameter and apparently filled the wellbore. Subsoil has apparently washed down the wellbore over time, creating the cavern.

Hole @ surface (2' x 2.5') was formed when lawn tractor was unknowingly run over void area. No gas leaks.
