The E&S controls were installed and appeared to be in good condition. The well pad was being constructed and is expected to be completed by mid October. A redline approval was granted to permit the construction of the detention basin during Phase 1 in order to reduce earth disturbance. Chevron indicated that the area around the access road is to be seeded and mulched the following week. Once the well pad is finished drilling activities may commence. There were no violations observed at the time of inspection.
2658460 Primary Facility 2017-11-06 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site was recently constructed, but vegetation was coming in on some portions of the site. Installed BMPs appeared to be in good condition. There were no signs of erosion and no violations observed at the time of inspection.

2768473 Primary Facility 2018-08-23 Routine/Partial Inspection No Violations Noted On 08/23/18 at approximately 1015 hours I met with Curtis Small of Chevron to discuss possible redline changes. The operator of an adjacent conventional well accesses their well from the Siegel well pad entrance. The conventional well operator had a preexisting access road that was modified and graveled in accordance with the approved E&S plan during construction of the Siegel well site. The conventional operator indicated to Chevron the access road as modified is steeper than it was originally. Chevron proposed reducing the grade of the road. Redline approval was granted to reduce the grade of the access road within Chevron's LOD.

2793177 Primary Facility 2018-10-17 Incident- Response to Accident or Event Violation(s) Noted On 10/13/18 at approximately 1600 hours Chevron had a synthetic based mud discharge from a flare stack in the southern corner of the well pad. The mud impacted the well pad surface, drainage channel, and storm water basin. Chevron ceased operations to prevent further fluid release. On pad material was removed and absorbent booms were placed in the basin.

On 10/17/18 I met with BA Lambie of Chevron and conducted an incident response inspection. The well pad had been excavated and sampling was being conducted. The majority of the drainage channel was excavated and the remaining portion was being completed. Chevron indicated they plan to drain and excavate the basin to prevent further impacts from oil droplets.

2846002 Primary Facility 2019-02-15 Compliance Evaluation No Violations Noted On 10/13/18 a synthetic based mud spill occurred at the site, resulting in the issuance of an NOV.

On 02/15/18 I conducted a compliance evaluation inspection at the site. The impacted pad slope, channel, and stormwater basin had been remediated. The remediated areas were properly stabilized and there were no signs of erosion observed. The violations have been corrected and there were no violations observed at the time of inspection.

2853076 Primary Facility 2019-03-13 Drilling/Alteration No Violations Noted This well has had the top hole drilled and cased and is tagged and inside a protective cattle gate waiting on the fluid Rig to arrive and finish drilling. There was no Drill Site Supervisor present on site at the time of this inspection. All equipment staged on site looked ok.

NOTE: I contacted Chevron and had a Compliance Specialist come to the site to darken the well sign information in a bit more clearly as the recent heavy rain made it a bit difficult to read.

2853843 Primary Facility 2019-03-13 Routine/Complete Inspection Violation(s) Noted On 03/13/19 DEP Soil Scientist Ed Bates was conducting an inspection to evaluate Act 2 compliance at the Siegel well site, due to a prior oil based mud spill. Bates observed the OBM impacted soil on secondary containment, but noted impacted soil had spilled, the impacted soil was no longer appropriately covered, and the secondary containment was insufficient. Bates contacted this WQS to inform me of violations observed at the site.

I contacted Curtis Small of Chevron to inform him of Bates' observations and that I was going to be conducting an inspection shortly. Small indicated he would also head to the site.

At approximately 1225 hours I, along with Water Quality Specialist Supervisor John Carson, conducted an inspection of the site. Oil & Gas Inspector James Braunns was also present at the site conducting an inspection prior to the fluid rig coming onto the site.

The impacted soil stockpile appeared to have been covered with tarps, but was now largely exposed. Spilled impacted soil was observed and
