On 02/02/16 at approximately 1400 hours I conducted a routine complete inspection of the Coastal Smith Unit. The Act 9 sign was in place and all required documents were present at the site. The flowback pit on site was approved by Department staff to be filled using fill from the site. A gravel pad was then constructed in the location of the filled pit without Department approval. This is a violation of 102.5(m)(4) for failure to perform work to the specifications of the ESCGP. |
