2451281 Primary Facility 2016-02-17 Administrative/File Review No Violations Noted On or about January 21, 2016, the Department received an ESCGP-1 major revision submittal for ESX10-051-0045. During the review process it was discovered that ESX10-051-0045 had already expired. Given that the permit was not terminated before expiration, the site is currently in violation of 25 Pa. Code §102.7(a). At the time the violation was identified, the operator was attempting to make a major revision to the currently expired ESCGP-1. The operator should consider submitting an application for a NEW ESCGP-2. The operator will still be required to maintain the Erosion and Sedimentation Control Plan in accordance with ESX10-051-0045 until the approval of the new ESCGP-2 is conferred by the Department.
2520163 Primary Facility 2016-08-11 Incident- Response to Accident or Event Violation(s) Noted

On 08/11/16 at approximately 1330 hours I conducted an incident response inspection of the Stevens drip tank in response to a spill that occurred on 07/22/16. The brine tank was punctured by a projectile, allowing approximately 8.3 barrels of produced water to spill onto the ground surface. The fluid is reported to be such a low concentration that it is capable of freezing, but no water samples were taken from the tank to confirm the concentration. XTO staff utilized a soil probe to test for the extent of the spill, but reportedly the soil readings were comparable to background. XTO is awaiting soil sample results from ESL labs. The following violations have resulted due to the spill:

2631026 Primary Facility 2017-08-03 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. XTO organized a meeting to discuss the restoration of the site and described the work to be completed. During the meeting I informed XTO that there is an outstanding violation from a spill at a drip tank associated with the site. XTO failed to enter in Act 2 or the alternate method and failed to perform a site characterization. XTO indicated they would initiate efforts to return to compliance.
