2753380 Primary Facility 2018-06-14 Complaint Inspection No Violations Noted On June 14, 2018 Justin Najewicz and I responded to a complaint at the Chevron National Mines Corp 26H well site located in Luzerne Twp. Fayette County. Curtis Small, Mike Herman, and Rodney Frazee were present from Chevron Appalachia LLC. The property owners were present. Ashley Funk from Mountain Watershed Association was also attended. The complaint is in regards to a seep in the pasture to the west of the well pad. The landowner contends that water in the seep has sickened his cattle and is the result of Chevron's Oil and Gas operations at the site. The landowner now prevents the cattle from accessing this pasture. This seep had been investigated and sampled previously. (See John Parry's 10/3/16 inspection report).

The seep has been developed with a small diameter pvc pipe. We collected a water sample from the pipe (#040). A soil sample was also collected nearby in the wet area (#041). We collected 3 additional water samples from around the property. #042 was collected from a spring fed wateri

2802582 Primary Facility 2018-11-14 Administrative/File Review Pending On November 14, 2018 I completed an administrative file review of Chevron Appalachia LLC's National Mines Corp 26H well site located in Luzerne Twp. Fayette Co. A DEP inspection report from 2/24/2011 was reviewed. The inspection was conducted by Beth Smock. At the time of the inspection, the National Mines Corp 26H well was operated by Atlas Energy Resources, LLC. Ms. Smock notes 4 pits onsite, all still lined, with water in them. One pit was being pumped out during the inspection. She also states the operator plans to use rapid hot flow technology to melt ice in the pits to pump water from them.

A well site restoration extension request dated February 11, 2011 was reviewed. This letter was submitted by James H. Braunns, Jr. Regulatory Compliance Manager for Atlas Energy. The letter discusses challenges in removing pit liners in a manner that would not compromise their integrity or cause a potential for pollution due to ice in frac pits at several well sites. 2833450 Primary Facility 2018-08-13 Joint Internal Site Inspection No Violations Noted On August 13, 2018 I conducted a joint inspection of the National Mines 26H well site. Mine Conservation Inspector Harvey Rock and his supervisor Andrew Walker were also present form the Department. Dep's Oil and Gas Program received a complaint (CTS Id 334091) regarding a seep down gradient of the well pad that according to the property owner was making his cattle sick. A response was made on June 14, 2018 and samples were collected from the seep and various other locations across the property. Dep's Mining Program also received a complaint (CTS ID 33148) about apparent mine discharges in the vicinity. A sample was collected from a seep along the driveway across the street from the access road, 176 Isabella Rd. This water had a strong rust color. Walker and Rock agreed that this seep was likely related to mine

discharge. The landowner was concerned about strontium in the seep on his property. According to Rock, the Surface Mining Program sample suite does not include strontium. I collected the sa

2840431 Primary Facility 2019-02-15 Complaint Inspection No Violations Noted On 2/15/19 a complaint inspection of the National Mines Corp 26H Well Site was conducted in response to a complaint (CTS ID: 339842). The following was observed:
