The wellhead appears to be 4 1/2" csg swedged to 2" with a closed valve. There does not appear to be any signs of oil, brine, or natural gas on location.
2785083 Primary Facility 2018-08-28 Compliance Evaluation No Violations Noted On 08/28/18 at approximately 1000 hours I, along with OGI Eric Peterson, conducted an inspection of the site. The site was overgrown and an alternate access route to the well had to be utilized. The site did not have production equipment and the well appeared to be abandoned, as large trees were surrounding the well.

On 11/06/2009 the Department issued multiple violations, including failure to maintain 2' freeboard in an impoundment and failure to restore the well site. At some point between 11/06/09 and 08/28/18 the impoundment was restored. While an E&S plan was not available, the site appeared to have been restored.

2814119 Primary Facility 2018-12-12 Follow-up Inspection Recurring Violations A follow up inspection was conducted, due to outstanding violations. The site appears consistent with photographs from inspections conducted on 08/28/18. The well head was still in place and had not been plugged. The violations cited on 08/28/18 remain outstanding.
