ON 5/22/2012 MAXIMUM’S CREW ENCOUNTERED A RELEASE OF FLUIDS DURING OPERATIONS OF SAND PUMPING ON 4 1/2" CASING. THE SAND LINE SOCKET FAILED WHILE EMPTYING THE SAND PUMP. THE LUBRICATOR AND SAND PUMP FELL OVER A FENCE RELEASING FLUIDS ONTO THE GROUND. TWO RIG HANDS VERIFIED THIS ON 5/22/2012 AT 4:30 PM. 2379753 Primary Facility 2015-05-20 Routine/Complete Inspection Outstanding Violations - No Viols Req'd On 05/20/15 at approximately 1330 hours I along with OGI Rick LEWIS met with Keyrock Energy LLC representative Mike BUCHEIT to discuss the outstanding violations and site characterization at the G. Harvey USX 4 well site. The site was fenced and gated to exclude horses from accessing the area of the spill and the well. The site was vegetated, but exhibited spots of bare, slightly rocky soil and areas of what appeared to be iron staining on the soil surface near the well head. There was slight erosion occurring near the well head going down into the fenced area. One yellow flag showing a previous sampling location was found at the site. After discussing the characterization expectations due to the spill being three years old, BUCHEIT contacted ESL labs to schedule sampling for 05/22/15.

2434308 Primary Facility 2015-12-03 Administrative/File Review Outstanding Violations - No Viols Req'd WQSS John Carson, EGM Dan Counahan, and I met with Chris Shea of Keyrock Energy and David Silbaugh of Silbaugh Hydrogeological Services to discuss the Harvey USX #4. It was agreed that five samples will be taken as a site charcterization in response to a 2012 spill at the site. The samples will be analyzed for the brine suite found in DEP Program Program Clarification Memo and will be taken as follows:
2731555 Primary Facility 2018-05-04 Complaint Inspection No Violations Noted Well appears equipped for production. Well head surrounded by security fence.

At time of inspection Department observed a 10'X10' area lacking vegetation (photo #1). This area is approximately 688' from the well head. This area surrounds an approximately 16" deep hole (photo #2). It appears that air/gas is escaping from this hole. Department contacted operator by phone from site. Operator rep Mark arrived and stated their brine collection line runs through this area. Mark also stated a water line and another gas line are located near this area. Department contacted Diversified Oil and Gas Co due to their ownership of a well on this property. Diversified Rep arrived on site and shut in their well. This did not cause the air/gas to stop escaping from the hole. DEP WQS Cindy Witham contacted DEP OGI Rick Lewis. Mr. Lewis arrived on site and recorded a reading of 100% natural gas with a 62s meter. Department requests operator ensure area is safe, replace/repair line, and remediate area per current regulations

2731584 Primary Facility 2018-05-08 Administrative/File Review No Violations Noted ADMIN inspection to inspection record #2731555.

Operator rep Doug Wadsworth contacted Department by phone, left message. Department returned Mr. Wadsworth's call later that morning. Mr. Wadsworth stated the leaking line was fixed yesterday (5-7-18). Mr. Wadsworth stated he feels water company struck line when they were working in the area. Mr. Wadsworth stated the excavated area is backfilled, seeded, and mulched. A follow up inspection will be conducted at a later date. Violation(s) may be forthcoming pending future inspections.

At time of inspection disturbed area appears to have been backfilled, seeded, and mulched. No leaking air/gas observed at this time. It appears the leaking line has been fixed. Inspection(s) will be conducted at a later date to ensure site is permanently stabilized. Violation(s) may be issued based on future inspection(s).

2733869 Primary Facility 2018-05-04 Administrative/File Review Violation(s) Noted ADMIN Inspection to inspection record #2731555. At the time of that inspection, it appears the buried line was compromised and air/gas was escaping.

Based on observations made in above mentioned inspection, a violation is being issued for venting of gas to the atmosphere that produced a hazard to the public health and safety. This inspection is being referred to OGI Supervisor for OGI follow up.

According to inspection record #2731584, operator reported that the leaking line was fixed on 5-7-18. Based on observations recorded in inspection record #2731589 no leaking air/gas was observed and it appears the line has been fixed. The violation issued on 5-4-18 has been corrected and is closed.

Department contacted operator rep Doug Wadsworth by phone. Department requested information about pipe lines located near area of no vegetation observed in above inspection. Mr. Wadsworth stated a municipal water line is located alongside the operators brine and gas lines. Mr. Wadsworth also stated that pictures were taken by him during the fixing of the pipe.
