2836299 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted This inspection was performed with Kurt Lewis of Hunt Marcellus to determine the proximity of a wetland to the disturbed area and to check on the overall condition of the pad and BMPs.

The wetland area was not marked out and thus it could not be determined how close it extended to the well pad. The access road was in good condition with stone and culverts were in good shape. The well pad and area around it are both stable, with 70%+ vegetative cover and no erosion concerns noted. Stormwater BMPs all appear to be functioning properly. The site ID sign and mailbox with permit and E/S inspection information are at the entrance to the site. No issues are noted on this site, which has no wells drilled on it.
