2628251 Primary Facility 2017-08-14 Routine/Complete Inspection Violation(s) Noted On 8/14/17 at 1012 hrs., I conducted a routine inspection of ARG's WT 3783 311 well (API 047-24218) in Highland Twp., Elk Co. The well pad is permanently stabilized. The well is properly labeled and equipped for production.

At the time of inspection, I also noticed fluid being discharged onto the ground from the wellhead. The fluid appeared to be coming from the backside of the well. Conductivity of the fluid was over 20 mS/cm. The flow rate at the wellhead was one gallon every 44 seconds. Over one barrel of fluid had been discharged, but it is unclear exactly how much fluid was released. I notified Tim Oknefski with ARG Resources of the release and he was onsite to shut the well in at approximately 1045 hrs. Tim notified me that the discharge appeared to be the result of a leaking packer.

The release of injection fluid onto the ground from a wellhead constitutes violations of 78.54, SWMA 301, and CSL 402(b). These violations will be resolved upon compliance with the Department's Spill Policy 800

2811134 Primary Facility 2018-12-04 Follow-up Inspection Outstanding Violations - Viols Req'd On 12/4/18 at 0844 hrs., I conducted a follow up inspection of ARG Resources, Inc.'s WT 3783 311 well site (API 047-24218) in Highland Twp., Elk Co. Of particular concern were the violations cited in inspection ID 2628251 relating to a release that occurred at the site. Impacted surface soil has been excavated and sample location markers are present along the spill path. Violation ID numbers 795308, 795309, 7953810, and 795311, from inspection ID 2628251 remain outstanding and are being cited again for this inspection.
