2619653 Primary Facility 2017-07-26 Incident- Response to Accident or Event Violation(s) Noted On 7/24/17, an unreported release of crude oil and injection fluid from a pipeline was brought to my attention by OGI Scott Motter. OGI Motter observed workers repairing the line leak across from ARG Resources' Gilbert 1034 well pad (API 047-23320).

On 7/26/7 at 1002 hrs., I conducted an inspection of the release site. Upon inspection, the release appeared to be over a barrel and residual fluid remained at the surface. The conductivity of this fluid was over the range of my conductivity meter (over 20 mS/cm). Mulch covered the disturbed area, which was roughly 15' by 8' at the surface.

The unreported release constitutes violations of 91.33(a) and 78.66(a). These violations have been entered and immediately resolved.

The release of crude oil and injection fluid onto the ground from a pipeline constitutes violations of 78.54, SWMA 301, and CSL 402(b). These violations will be resolved upon compliance with the Department's Spill Policy 800-5000-001.

2792913 Primary Facility 2018-10-23 Compliance Evaluation No Violations Noted On 10/23/18 at 0906 hrs., I conducted a Compliance Evaluation Inspection of the release location adjacent to ARG Resources, Inc.'s Gilbert 1034 well site (API 047-23320) in Highland Twp., Elk Co. The release was documented previously by inspection ID no. 2619653 and associated NOV. On 10/10/18, the Department approved the Remedial Action Completion Report (attached) for this release. Upon my inspection today, the release site has been restored and permanently stabilized. This inspection resolves violation ID no. 792555, 792558, and 792559 from inspection ID no. 2619653. There are no additional violations noted for this inspection.
