2029692 Primary Facility 2011-12-27 Routine/Complete Inspection No Violations Noted Upon inspection today I found a new well access road and well pad under construction. The Site ID- Permit and E&S Plan where posted. The access road was stoned; the well pad was being stoned while on-site and the pits being dug. One item of concern was that a top soil pile was approx 15' from a blue line stream w/o BMPs to control run-off. I called Drilling Supervisor, Jeff Foley and explained that BMPs to control potential run-off must be installed right away; he agreed to have the dirt contractor install whatever was needed. All setback requirerments have been met.
2029881 Primary Facility 2011-12-28 Follow-up Inspection No Violations Noted Today's inspection was in follow-up to my concerns identified during my 12/27/2011 inspection, and to evaluate the current status of this new well site. Upon inspection I found the items of concern I had mentioned to the Drilling Supervisor, Mr Foley had been addrssed. BMPs have been installed as requested. The well pad is still under construction The Site ID- Permit and E&S Plan where posted. The access road condition has in good condition. the well pad was stoned. No rig on-site.

The access road was in good shape. Both pits had over 4 ft of freeboard. Some sections of the slit fence needed repaired. I called the Jeff Foley, Project Manager, and he agreed to have the BMP repaired today.
