2637595 Primary Facility 2017-09-18 Routine/Partial Inspection No Violations Noted I arrived onsite with Clark Smith of the Department's Environmental Cleanups Program to observe the drilling of a monitoring well on the southwest side of the emptied impoundment. John Eisel of Moody and Associates, Inc. was present; well was being drilled by Eichelbergers. I noted several drums on the pad in a lined earthen containment that were simply labeled "non-hazardous waste"; these drums should be labeled to reflect the actual contents. We remained on the site until the rig reached approximately 25' depth.

Routine inspection of Greylock Energy, Cold Stream Affiliates B Pad, well number 5MH-S on February 20, 2018. There is one producing wells on this pad. Two annulus vents (9.625 in. x 5.5 in.) are closed and not vented at time of inspection. Annulus vent (13.37 in. x 9.625 in.) is open and vented; 0% combustible gas was detected at the annulus vent. The production Xmas tree top pressure gauge reads 127 psi. 0% combustible gas was detected in the area during inspection. Wellsite looks neat and well organized. Observed 4 metal barrels filled with flammable liquid off the containment, followed up with company representative via the phone regarding solving this issue. No violations noted at the time of inspection. DEP Representative ¿ Ulyana Trynovich, Oil and Gas Inspector.

2722700 Primary Facility 2018-04-24 Routine/Complete Inspection Violation(s) Noted No activity on the site when I arrived. Former produced fluid pit was still open, and some water had collected at the bottom; conductivity of the water was from .067-.11 mS/ cm. The single well on the site is in production. The well tag still has Energy Corporation of America listed on it; I recommend that this tag be updated. A small metal tank was present between the production tank and the compressor unit. I noted 2 spots at the bottom of the tank where the gravel was stained; the stained areas had a slippery feel, and had a slight odor of petroleum and glycol. Although these spots were not large, the tank did appear to be leaking, and one of the spots had visible corrosion at the bottom corner of the tank. I immediately contacted JD Sollon of Greylock; he informed me that he no longer worked in the area but would contact someone who did. A short time later, Roy Rickert of Greylock called me, and I informed him of my findings. He said that the tank is a dump tank for the compressor, and that the
2725781 Primary Facility 2018-05-03 Administrative/File Review No Violations Noted This administrative inspection was generated regarding Greylock's response to the Notice of Violation for the site from a 4/24/18 inspection. Greylock's NOV response is attached, as well as pictures documenting the investigation and cleanup. The response indicates that, rather than the tank being breached, the staining occurred due to high dump pressure to the tank occasionally creating a mist that settled on top of the tank, and subsequently ran down the side of the tank during precipitation events. The cleanup pictures show that the staining was shallow, supporting this conclusion. To prevent a similar issue in the future, Greylock installed a choke nipple on the dump line to reduce both overpressure and volume of fluid being dumped.
       The Department appreciates  quick and thorough response regarding the NOV.  The violations related to the release from the dump tank have been closed, and no followup is requested.
