No one was onsite during my inspection. I later spoke with JD Sollon and Travis Wendel via phone on December 4, 2017 regarding some questions I had about this pad and ESCGP. Both gentlemen informed me that ECA was recently bought out by Grey Lock Energy (GLE). GLE is in the processing of having their well permits transferred from ECA to GLE. Both gentlemen are now employed by GLE.

The Whitetail #1 Pad has two producing Marcellus wells located on it (033-26961 and 033-27086). Both well cellars are full of water and no sheen was obsereved. I observed bubbling within the well cellar of 033-26961. I recommend the operator further investigate the bubbling. I will be referring this bubbling to one of the Departments Oil & Gas Inspector's for further investigation.

Follow up inspection of Energy Corporation of America, Whitetail 2MH Pad, well number 3MH on December 8, 2017. Annulus 9-5/8" was plumbed to the production line at time of inspection. Digital gauge on the production tubing reads 46 PSI pressure. 0% combustible gas was detected in the area immediately surrounding the well. Used plastic jug to capture bubbling and 4% LEL combustible gas was detected at the time of inspection. This was followed with pumping the water out of the cellar by vacuum truck. Visibly confirmed unknown combustible gas escaping the ground from the outside of the well casing. No violations noted. Discussed follow up inspection with company representative present on site. Annulus 9-5/8" was plumbed to the poly tank at time of inspection. Digital gauge on the production tubing reads 44 PSI pressure. 0% combustible gas was detected in the area immediately surrounding the well. Visibly confirmed unknown combustible gas escaping the ground from the outside of the well casing. No violations noted. Discussed follow up inspection with company representative present on site. 2711059 Primary Facility 2018-03-27 Follow-up Inspection No Violations Noted The weather is 40 Deg. F and cloudy, windy at time of inspection.

Follow up inspection of Greylock Production LLC., Whitetail Pad, well number 3MH on March 27, 2018. There are 2 active wells on site. This well is producing. Annulus (9-5/8" x 5-1/2¿) was plumbed to the poly tank at time of inspection. Digital gauge on the production tubing reads 42 PSI pressure. 0% combustible gas was detected in the poly tank or the area immediately surrounding the well. Visibly confirmed there is no combustible gas escaping the ground from the outside of the well casing during inspection. No obvious concerns or violations are noted at time of inspection. 2764475 Primary Facility 2018-08-14 Routine/Complete Inspection No Violations Noted Department staff Sue Herzing, Sara Metz, and Mike Jurbala conducted a routine site inspection at Greylock Production LLC Whitetail #1 well pad and flowback impoundment on August 14, 2018. The operator is currently in the process of dewatering the flowback impoundment. Water is being hauled offsite via tank trucks. During the inspection we observed two holes within the flowback impoundment liner near the load-out area (photo 4). Another layer of liner was observed underneath the holes.

Greylock is currently in a Consent Order and Agreement (CO&A) with the Department finalized in November 2017. According to the CO&A the operator is required to restore this flowback impoundment and conduct site characterization, consisting of a sub-liner investigation after the liner is removed. If issues are found during that sub-liner investigation, then the operator shall conduct a site remediation in accordance with ACT 2.

2786646 Primary Facility 2018-10-09 Compliance Evaluation No Violations Noted I met with JD Sollon of Greylock on the site. Two skid steers and a 3- man crew were removing the liner and felt from the on-site impoundment. Moody & Associates group on location included Brandon Glowark, Ian Williams, Jona Stoskey, and Brynn Yochim. Moody was there to perform soil screening after the liner removal was completed; screening was to be for specific conductance, petroleum product, and rads. Although it appeared initially that the screening would begin while I was still on location, one of the skid steers threw a track and was being repaired until I departed. I noted several freshwater seeps on the northern slope of the impoundment, which were collecting mostly in the western side. The plan was to dig an interceptor trench across the north and west sides of the impoundment at the bottom to collect these seeps. Then Moody would perform the soil screening and take characterization samples. Once the liner was removed from the bottom of the impoundment, I walked the area taking soil conducta
2811476 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Greylock Production LLC Whitetail flowback impoundment and well pad on December 6, 2018. The flowback impoundment has been completely reclaimed, seeded, and mulched. In addition, the well pad has been reduced in size on the S & SE sides. Both restored sides have been seeded and mulched. E&S BMPs and PCSMs remain in place and appear to be functioning as design.

Two Marcellus wells remain in production on the well pad. Well cellars are full of water and are frozen. Each well is connected to (2) 400 bbl produced fluids tanks which are stationed within a clay lined earthen secondary containment. Frozen water ~ 8-10" was observed in the earthen containment. A skid mounted compression and dehydration unit is stationed onsite. Gas leaving this Whitetail #1 pad is being fed to the NRG-Shawville Plant via a modified ECA transmission line.
