2572624 Primary Facility 2017-03-17 Routine/Complete Inspection Violation(s) Noted Inspection of CNX’s Hartzfeld 9 (033-26740) conventional gas well in Brady Twp., Clearfield County conducted March 17, 2017 at 10:30 am. Yesterday the operator called to report gas noted outside the 7” csg. Today we met on location to go over the well’s condition, see where integrity issues are noted, and discuss any possible migration pathway to surface. At the time of inspection; 13 3/8” x 7” annulus was open to atmosphere with audible bubbling. The 7” x 4 ½” annulus was capped with a hammer union, (1) side outlet was open to atmosphere with notable flow of gas. The 4 1/2” csg was open to production line (~50 PSI) through side outlet. The well was previously producing through (1”) tbg string which is now shut-in/isolated from production line at a ball valve on the wellhead. The 4 ½” production casing was shut-in during that time. Integrity issues were noted after the 4 1/2” casing was opened-up and turned inline to production.

At the time of inspection; the well was shut-in from production with ~120 PSI on the 4 ½” csg, 25 PSI on 1” tbg. The 7” was open and venting at ball valve, hooked up gauge, 1-2 PSI. There was audible bubbling in the annulus between the 7” and 13 3/8”. No gas detected outside the 13 3/8” at this time including at the monitoring well ~50 yards from the well. Violation of 25 PA Code Ch. 78.85 outstanding.
2577168 Primary Facility 2017-03-29 Routine/Complete Inspection Outstanding Violations - Viols Req'd Inspection of CNX’s Hartzfeld 9 (033-26740) conventional gas well in Brady Twp., Clearfield County conducted March 29, 2017 at 11:30 am. At the time of inspection; the well was in production, 1” tbg was shut in, 4 ½” production casing was open to sales at side outlet. The backside of the production casing was open and venting through 2” ball valve. There was no audible bubbling in the annulus between surface casing and conductor. I tested the annulus outside the 7” surface casing for gas with an Altair 5x gas meter: 0-1% gas detected below the casing and no gas detected above the casing top. The 7” was venting a minimal amount of gas, considerably lower than it was venting last week. I spoke with the operator who said there was ~250 PSI on the well last Thursday, they opened the 4 1/2” back up to sales and haven’t heard bubbling since. Still waiting to hear from operator on how apparent defective cement outside the 7” has or will be remediated. 78.85 Violations outstanding
2580297 Primary Facility 2017-04-05 Routine/Complete Inspection Violation(s) & Outstanding Violations Inspection of CNX’s Hartzfeld 9 (033-26740) conventional gas well in Brady Twp., Clearfield County conducted April 5, 2017 at 11:00 am. At the time of inspection; the well was in production, 1” tbg was shut in, 4 ½” production casing was open to sales at side outlet. The backside of the production casing was open and venting through 2” ball valve. A field reading with an Altair 5x gas meter showed 3% methane at vent. Flow was measurable but minimal; I put a balloon on the vent and inflated in 1 minute, but not enough flow to blow the balloon up or off thereafter. The annulus between the surface casing and conductor was open. I tested the annulus outside the 7” surface casing for gas with an Altair 5x gas meter: The meter showed 4% methane in the annulus below ground level and 2% methane at ground level. Gas (15% LEL) was detected outside the uncemented conductor, in violation of 25 PA Code § 78.73. The backside of production casing is open at a 2" valve. The wellhead is on top of the surface casing, and the backside of surface casing (x conductor) is a cemented and open annulus. I checked the surface x conductor annulus for gas with an altair 5x gas meter and monitored for 10 minutes. No gas was detected at any time during the 10 minutes. I checked the production x surface casing annulus for gas at the open valve and monitored for 10 minutes. No gas was detected at any time during the 10 minutes. There was no smell of gas or any obvious visual concerns. No violations were observed during this particular inspection.

Annular vent (7" x 4-1/2") was closed upon arrival to the wellsite. Company operator has opened annular vent during inspection, 0% combustible gas was detected at the annulus and in the area immediately surrounding the well. Verified pressures: tubing ¿ 25 PSI, backside casing ¿ 26 PSI, annular vent ¿ 0 PSI. Production gas sample has been collected by GAS Analytical representative David Yewcic, Meter Technician and Field Admin. Annulus vent has been shut in approximately 30 days prior today's inspection. 2856183 Primary Facility 2019-03-20 Administrative/File Review Violation(s) Noted ADMIN inspection created to cite violations to enter an Enforcement Record (NOV) against Diversified Gas and Oil, PLC (Alliance Petroleum Corporation) and the Hartzfeld 9 gas well (033-26740) related to a gas migration investigation/water supply impact in Brady Township, Clearfield County.

Violations were previously cited against the Hartzfeld 9 and its previous operator (CNX Energy) for the presence of combustible gas outside of surface casing and outside of conductor casing. CNX was to plug the well, but neither plugged the well nor corrected the defective cement. Consequently, the well is now considered Abandoned and the operator has failed to plug an Abandoned well.

This inspection/enforcement requests that Diversified submit a corrective action plan to address the defective cement at the Hartzfeld 9 gas well, provide temporary water to the owners of the impacted Water Supply within 24 hours, and submit a plan to permanently restore or replace the impacted Water Supply.
