1800929 Primary Facility 2009-05-15 Routine/Complete Inspection Violation(s) Noted Site and existing enforcements are being transferred from WQS Curtis Small to Jessica Miller. Site was visited on 13 May 09 with concerned neighbor. Site was visited again on 15 May 09 with OGI Bruce Jankure and WQS Supervisor John Ryder. Access road remains unrestored. The well pad has been recontoured, seeded and mulched. All fill material used for the access road and well needs to be removed. The area must be restored. A restoration plan needs to be submitted.
1804971 Primary Facility 2009-06-11 Complaint Inspection Violation(s) Noted Site visit in response to complaint of cloudy water running in East Branch Mohoning Creek. Discoloration was traced to an unnamed tributary to E.B. Mohonning Creek and up tributary to Fawcett #2 well pad. At point of previous unpermitted crossing, there were tire treads on either side of the road, leading into the stream. Site was being restored, fill removed and area was seeded and mulched. No extra BMPs were in place at crossing to prevent E&S problems. sediment deposits were visible just down stream. Dan Sinclair of Dannic Energy reported that all fill was removed and some topsoil was placed down prior to seeding and mulching. Culvert was supposed to be place at stream crossing temporarily and then removed when work was complete in that area. E&S plan should have been on site with restoration team. Operator was asked to submit the restoration plan used to restore the site and place proper identification sign and E&S plan on site.

Well will not be drilled, Pit and pad have been backfilled, seeded and mulched. Stream Crossing has been removed. Site is not fully restored. No E&S plan or identification sign on site. area were pit was created is beginning to sink. Silt fence has been installed in waterway and not properly jointed. sediment bar forming just downstream of former crossing. well pad and access road constructed in forested wetland greater that 1 acre in size. Waiver for distance requirements has been submitted. Operator was also asked to submit restoration plan for approval prior to removing any fill from access road.
1807690 Primary Facility 2009-06-25 Follow-up Inspection Outstanding Violations - No Viols Req'd Site inspected with John Ryder. As of inspection on 11JUN2009, the following improvements have been made: Filter fence has been properly reinstalled on both sides of former crossing and mulch has been cleared out of stream. Sediment from access road is no longer entering stream. Any area where concentrated waterflow had been stopped by the filled access road has been rechannelled, allowing water to flow freely accross disturbed area. Restoration activities have ceased and no further disturbance should occur at this site. Seed and mulch cover the entire disturbed area. Vegetation has begun growing. There is no evidence that wetland species of vegetation or hydric soils were used in restoration of the site. However, walking along the former access road was difficult due very wet soils, indicating that the hydrology is being re-established.
