1808881 Primary Facility 2009-06-25 Drilling/Alteration De minimus Violations Noted Site visited with John Ryder. E&S plan had been altered and initialed by company representative but not by a DEP rep. There was also extra silt fence and a large topsoil pile that were not part of the original plan nor documented as changes. Existing silt fence had been punctured by brush row. Water is channelled directly to silt fence, has followed silt fence, picking up energy and eroded a small pocket out from around downslope stakes, where it exited the site. No evidence of sediment being carried off site at this point. Pit is being constructed, contains approx. 1.5 ft of water, straight walls with jagged edges, no liner yet. diversion ditches on site are unlined and filling with sediment. Could potentially cause a pollutional event. More details on hardcopy inspection report. Company has been asked to correct deficiencies. DVN code used because no pollutional events have occurred and site is not completely constructed yet.

1822357 Primary Facility 2009-08-13 Drilling/Alteration No Violations Noted Site was visited with OGI Bruce Jankure. Well still not drilled yet. While speaking with Mr. Huey at site 4H, we learned that the permit for 6H has expired and activity has ceased until permit is renewed.
