Clearfield County conducted 12/15/16 at 12:50 pm. It is a conventional well, in production. At the time of inspection; Location was snow covered. Wellhead is on the 7” csg. I dug out around the wellhead, 7” csg (x 9 5/8”) annulus is open, and looked like it had some frozen gel on top and around the wellhead. It was well below freezing; no flow from the annulus observed and nothing observed that needed immediate attention at time of inspection. No violations were noted during the inspection.
2548105 Primary Facility 2016-12-27 Routine/Complete Inspection Violation(s) Noted Inspection of the Hartzfeld 5 (033-26533) well in Brady Twp. Clearfield County conducted 12/27/16 at 1:45 pm. The entrance gate was locked; I walked about a quarter mile down the lease road to the well. About 60’ from the well I noticed the lease road was bubbling. The bubbling on the ground was vigorous approaching the well, and a gas check in the air above the bubbling with an Altair 5x gas meter yielded 100% LEL 100% Methane. Approaching the well; the wellhead was on the 7” the area around the 7” was full of water with vigorous bubbling. Violations cited from 25 PA Code Chapter 78 include 78.81 & 78.85 Casing and Cement given the obvious well integrity issue.

Clearfield County conducted 12/29/2016 at 10:15 am. The Department met with Consol onsite. At the time of inspection; pressure had been bled off the nearby gathering pipeline. The ground was covered in snow and gas was still present on the lease road approx. 50-60’ from the well itself. The Hartzfeld 5 was shut in with ~60PSI at the wellhead. The cellar had bubbling and you could hear the gas gurgling down in the cellar below the snow, ice, and water outside the 7”. The migration pathway to surface was discussed but nothing conclusive or confirmed. We discussed killing the well with water and gel. Consol will then likely plug the well. Samples were taken from the lease road, in the cellar outside the 7”, and a production sample from the wellhead for analysis and troubleshooting. Violations Outstanding

2549722 Primary Facility 2017-01-04 Routine/Complete Inspection Outstanding Violations - Viols Req'd Routine Inspection of the Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted 1/4/17 at 11:30 am. There were no significant changes noted at the well or location since last inspection. One thing I noticed this time was when I was walking down the lease road to leave location I discovered a monitoring well in the woods off the side of the lease road roughly 400-450’ from the well. The well was about 7” PVC about 2-3’ above ground with a crack and a metal well cap on top. The water level was well below surface and you could hear bubbling, I detected 16% methane in the well. I notified the operator of my findings. Operator plans to kill the 5V with gel as soon as possible and plug the well thereafter. 78.81 and 78.85 Violations Outstanding.
2553039 Primary Facility 2017-01-13 Plugging(Includes Plugged/Mined Through) Outstanding Violations - Viols Req'd Routine Inspection of the Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted 1/13/17 at approx. 12 pm. There were no significant changes noted at the well or location since last inspection; cellar, lease road, and monitoring well are bubbling at the same rate and no new surface expressions of methane were observed on the property. The operator plans to mobilize equipment within a few days and plug the well thereafter. I met with the landowner on location and we discussed the history of the well. The rig crew pulled up in a pickup while we were talking to locate the well and scope out the location for plugging operations which will likely start next week. 78.81 and 78.85 Violations Outstanding.
2553094 Primary Facility 2017-01-17 Plugging(Includes Plugged/Mined Through) Outstanding Violations - Viols Req'd Inspection of the Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted 1/17/17 at 10:30 am. The companyman and Universal cementers were staged at beginning of the lease road waiting on water trucks for well kill. I drove down the lease road to the well. A service rig was rigged up on the well ; I spoke with the crew, they said there was 60 PSI on the well upon arrival and blew down the well to tank. I took a look at the well and cellar. They cellar was pumped down; now I could see the bubbling was coming from outside the 9 5/8” casing. The water trucks showed up. Universal spotted equipment and bullheaded 70 bbls of gel. Cellar and lease road still bubbling at same rate. The rig tagged bottom w/ cable at 7855’. The rig will run open 2 3/8” tubing to bottom and cement with Universal. I went over volumes and calculations for plug job w/Companyman and Universal. They’ll set (2) plugs later this evening; Class A on bottom, LD 21jts tbg and set 50/50 pozmix plug #2 on top, WOC o
2553098 Primary Facility 2017-01-18 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted 1/18/17 at 10:15 am. Rig crew only on location. They were finishing up pulling last joint of tubing after WOC overnight after cement plug job last night. The cellar was full of water and there was no bubbling noted. There was no bubbling noted on the lease road anymore as well. The went in w/cable and tagged last night’s plug at 1365’. There were (2) slurry samples from the job next to the well; The Class A sample was showing good strength, the 50/50 pozmix sample was soft, still transitioning but showing some strength. I put 7” plastic bottle with hose hooked up to an Altair 5x gas meter right on top the 7” casing and did not detect any significant amount of gas. I took a walk to a monitoring well ~500’ foot from the well and detected 8-10% methane, down from 80-90% that I detected before operations. I called the Companyman from location to tell him about my findings. He said we was working on getting a
2553845 Primary Facility 2017-01-19 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted 1/19/17. I arrived on location at 10 am. There is a rig on location for plugging ops, Universal Cement crew spotted in for plug job. The rig tagged top of previous plugs yesterday at 1365’. Universal just got done pumping 1st plug on top of that point; 27 bbls of 50/50 pozmix. I spoke with the companyman and reviewed the bondlog they ran on the 7” casing yesterday. The log showed ok cement bond from the shoe at 884’ to ~200’ and some ratty cement above that point. 9 5/8” set at 308’. The rig pulled was pulling 19 joints of tubing. Universal came on line and pumped last 50/50 pozmix plug. They broke circulation right away and we saw cement returns after 27 bbls away. Universal shut down, the rig pulled tubing, Universal threw a kelly hose in the 7” and topped off w/pozmix to surface the volume previously displaced by tubing. They topped off to surface and there was some fallback at first, continued to top
2553847 Primary Facility 2017-01-20 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted 1/20/17. I arrived on location at approximately 10 am. There was a steady rain. The rig and equipment were rigged down and off location. There was a monument 4’ high cemented in the 7” casing with api number. The cellar is still dug out, partially filled with water outside the 9 5/8” casing where it was previously bubbling before the job. The bubbling has ceased and there was no gas detected in any amount in the cellar with an Altair 5x gas meter. Cement to surface was visible inside the 7” casing, still slightly soft and there was no bubbling observed or methane detected. The backside of the 7” (x 9 5/8”) annulus was visible with some soft cement caked on top there from yesterday’s job with no visible bubbling and no gas detected. There were 3 slurry samples on location; 1) Class A, 2) 50/50 pozmix and 3) 50/50 pozmix from yesterday, no irregularities noted. There was a lot of standing water on the pad/
2555833 Primary Facility 2017-01-26 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Hartzfeld 5 (033-26533) plugged conventional gas well in Brady Twp. Clearfield County conducted 1/26/2017 at approximately 11 am. It was 36 degrees with a snow shower. The access gate was locked; I walked in about a quarter mile down the lease road to the well. The lease road was very wet and muddy. I did not observe any bubbling on the lease road. The 5H had the 4” surface monument in place which was cemented in the 7” csg and the cellar was now filled in with gravel. There was no audible bubbling and I could not smell any gas. I put a plastic jug with hose hooked up to my Altair 5x gas meter on the ground on top of the gravel filled cellar above where the well was previously bubbling outside the 9 5/8” casing. I monitored the meter for ~10 minutes. No gas in any amount was detected. I walked 50’ from the well to the spot where the road previously was bubbling before the job. 2560720 Primary Facility 2017-02-10 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of the plugged Hartzfeld 5 (033-26533) conventional gas well in Brady Twp. Clearfield County conducted February 10, 2017 at approximately 11:00 am. At the time of inspection; the location was snow covered. I shoveled around the monument above the gravel-filled cellar and probed the gravel with a metal probe/hose hooked up to an Altair 5x gas meter. No gas in any amount was detected. I measured 50’ from the well to the spot where the ground was bubbling before plugging operations, shoveled, placed plastic jug down and tested for gas with Altair 5x gas meter; no gas. I walked to the monitoring well 500’ from the well and placed jug inside the plastic casing and tested for gas, monitored for 1 minute; no gas in any amount was detected.

2784908 Primary Facility 2018-10-05 Site Restoration No Violations Noted The Department received a well site restoration report on July 24, 2017 submitted by Consol Energy for plugged well 033-26533.

Department representative Sue Herzing conducted a well site restoration inspection at plugged well 033-26533 on October 5, 2018. The plugged well is represented will a well monument labeled with the well name, API #, and operators name. The area surrounding the plugged well is very well vegetated. No E&S issues were observed. The site is stable.

Per this inspection the Department approves the well site restoration report.
