2596659 Primary Facility 2017-05-22 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman’s Carl E Harkleroad #4 conventional gas well in Bell Twp., Clearfield County conducted May 22, 2017 at approximately 11:30 am. The plugging rig is off location. The well had surface casing above ground level, no monument installed yet. The casing was full of gravel from top of last plug with some water on top. There were some small bubbles noted at the time of this inspection with minor sporadic bubbling inside the casing where the vent will be installed. No gas was detected at the top of the casing using an Altair 5x gas meter, and no gas detected outside the casing. There were no major issues noted during this inspection, however a future inspection may be necessary to verify.
2604474 Primary Facility 2017-06-13 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman’s Carl E Harkleroad #4 conventional gas well in Bell Twp., Clearfield County conducted June 13, 2017 at approximately 12:00 pm. A rig is spotting in to drill out past cement plug past shoe to run in with camera to troubleshoot cause of bubbling. Suspected source of bubbling is shallow gas show near the shoe. I reviewed the open hole log on location with the operator and discussed the procedure. No major issues.

Rig and crew on location with camera. They ran downhole, bubbles reportedly coming from below the shoe, not at any casing collars. They are planning to pump cmt next week to squeeze channel to fix the issue.

Rig and crew on location waiting on bit. I spoke with the crew to discuss plan going forward. The operator intends to drill out 8 5/8” casing past the casing shoe and make another attempt at cement plug(s) in the openhole.
2625973 Primary Facility 2017-08-15 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman’s Carl E Harkleroad #4 conventional gas well in Bell Twp., Clearfield County conducted August 15, 2017 at approximately 1 pm. The well had 8 5/8” x 4” swage with 4” x 2” crossover and open ball valve, visibly venting and audible bubbling at the vent. Cement samples on location looked ok. The operator is attempting to plug is well, shallow gas show near casing shoe is suspected to be causing the issue.
2662422 Primary Facility 2017-11-17 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location, in process of rigging down equipment following another plugging attempt. I spoke with the driller, they drilled out past the casing shoe at 766', drilled down to 870' and cemented again. At the time of inspection, the 8 5/8" casing was open. I checked inside the casing with an altair 5x gas meter. Approximately 5% gas was detected. The annulus outside the casing had standing water. There was no bubbling observed. There was no audible bubbling or flow observed inside the casing.
2683732 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted Santo Adamo (production supervisor) of Fairman Corporation reached out to me on January 16, 2018 regarding E&S controls at their recently plugged conventional well Carl E Harkleroad 4 (033-23420). This well was finished being plugged in November 2017. According fo Mr. Adamo, once the weather breaks in the Spring of 2018, the operator will perform final restoration work.

I conducted a site inspection at conventional well 033-23420 on the afternoon of January 18, 2018. The ground was snow covered. Onsite was an E&S plan and PPC Plan. I observed a row of silt fence down gradient of the plugged well (photo 1). I observed a line pit that was snow covered (photo 3). During the time of my inspection, I was unable to determine if any residual waste materials remain present within the lined pit.

I will be conducting another inspection once the snow melts to determine if any further E&S controls are needed.

2687484 Primary Facility 2018-01-25 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a well being plugged. Operations have temporarily ceased this winter. At the time of inspection; well is venting through swage, valve, 2" pipe. Gas was present at the vent. I placed my hand over the vent, no significant pressure felt after 30 seconds. No obvious major issues were observed.
2771957 Primary Facility 2018-09-06 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this conventional gas well was conducted September 6, 2018. Plugging of this well has been ongoing since last year; venting was observed after initial plugging and there has been an effort to identify the suspect source of gas, drillout, and re-cement thereafter. After further remediation was considered unsuccessful, the operator has vented the well until recently, where they are now rigged up for another attempt to finalize plugging. The suspect source of gas has been determined to be near the 8 5/8" casing shoe. At the time of inspection, a rig is on location. No personnel were on location. There were (4) cement samples on location; (3) of which were slurry samples still transitioning and (1) which is set up. The well itself has 8 5/8" x 4" crossover and 4" x 2" swedge with a valve open to atmosphere. There was no audible or visible venting at the time of inspection. It appears they are WOC after cementing earlier today. No obvious major issues were observed.
2772835 Primary Facility 2018-09-10 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this conventional gas well was conducted September 10, 2018. At the time of inspection, there is a rig is on location for for final plug and abandonment. No personnel were on location. The well is open to atmosphere. There was no audible or visible venting at the time of inspection. No gas was detected with an Altair 5x gas meter. No obvious major issues were observed.
2777133 Primary Facility 2018-09-19 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged conventional gas well was conducted September 19, 2018. At the time of inspection, the rig and all equipment are off location following most recent of several attempts to plug this well. A 4" vent with marker plate has been made up to 8 5/8" surface casing as a plugged well monument. There was no visible venting. Traces of methane were detected near the mouth of the vent with an Altair 5x gas meter. No obvious major issues were observed during this inspection.

2804287 Primary Facility 2018-11-16 Routine/Complete Inspection No Violations Noted A routine, post-plugging, inspection of this recently plugged conventional well was conducted November 13, 2018. At the time of inspection, all equipment is off location. A 4 1/2" vent pipe has been installed on 8 5/8" casing as a plugged well monument. The 8 5/8" x 11 3/4" annulus was observed near ground level; no issues were observed. There was standing water outside of the 11 3/4" casing; no bubbling. No obvious issues were observed.
2833637 Primary Facility 2019-01-29 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, follow-up inspection of this plugged conventional well was conducted January 29, 2019. At the time of inspection, the location was snow covered. There is a 4" vent pipe installed on the surface casing as a plugged well monument. There was snow in the area surrounding the base of the monument, outside of surface and conductor casings. The snow was clean; no evidence of discharge was observed. No evidence of discharge was observed below the mouth of the vent. No open holes, bubbling, visible venting, or other major obvious issues were observed.
