2804496 Primary Facility 2018-11-15 Routine/Complete Inspection No Violations Noted Routine site inspection of Greylock's COP 326 #6 conventional well on November 15, 2018. This well permit was issued back in 1982 to Adobe Oil & Gas and the vertical well was spud and stimulated in the Oriskany formation in November/December 1984. ECA submitted a revised completion report to the Department in 2006. ECA documented a re-perfing and fracing of the Devonian shale interval (Marcellus) in March 2006. Since then, the well has been transferred to Greylock. The well is in administratively "active" status, but appeared shut-in during the time of the inspection. Looking at production reports over the past twelve months, the well has only been in partial production for the months of June and July 2018.

The area surrounding the well and production equipment has been restored. The Department approved a well site restoration report on 3/21/16. No ESCGP was ever issued for the construction of the well pad.

No violations noted.
